The journal disseminates information on the current state and future of Russia’s social policy for people with disabilities, the practices of Russian regional and territorial agencies working in the field of social protection, and modern approaches to solving the problems of disabled people in other countries. Special attention is devoted to original research focused on new promising methods of diagnosis and treatment of disabling conditions.

Correlation between the characteristics of the spheres of vitality and the gerontological profile in elderly patients
Abstract
ВACKGROUND: The study is devoted to the study of the relationship between resilience and gerontological status of elderly women to determine the prognostic factors of successful aging. Resilience includes engagement, control, and risk-taking. Gerontological status encompasses physical, psychological, and social well-being.
АIM: Analysis of the examination results of 70-year-old patients, taking into account the domain structure and concomitant diseases.
MATERIALS AND METHODS: The study was conducted in the departments of medical prevention from January 2017 to December 2024, the clinical base of the Burdenko VSMU. The parameters of somatic status, locomotor, sensory, cognitive, psychological and nutritional spheres were analyzed. The following methods were used: Cardiovisor, echocardiography, HRV analysis, determination of NLR, PLR, glycated hemoglobin, CRP, omega-3, FRAIL, Katz, MNA scales, dynamometry, bioimpedance, ophthalmological examination, MMSE, Beck and SHAS scales. The data was processed using Startech 4.8.0.
RESULTS: The high prevalence of risk factors such as lack of physical activity (62%), hypertension (64%) and chronic heart failure (66%), combined with alarming indicators of presarcopenia (30%) and sarcopenia (26%), indicate a complex impact on the health and functionality of the subjects. The high average systolic blood pressure (175.00 mmHg), the prevalence of obesity (BMI 37.10 kg/m2) and hyperglycemia (glucose 10.60 mmol/L) underline the urgent need to develop and implement comprehensive prevention and treatment strategies.
CONCLUSION: The evaluation of the presented results makes it possible to establish priority areas for the creation of individualized prevention and treatment plans aimed at reducing the likelihood of developing cardiovascular complications, maintaining mental abilities and improving the overall well-being of the studied group.



Critical points of viability in elderly patients with visual deficits
Abstract
BACKGROUND: With an increase in the average life expectancy of the population, the number of elderly people is increasing, and issues related to their health and vitality are becoming more relevant. Critical points of viability in the elderly are the moments when their body becomes the most vulnerable and requires special attention and care.
AIM: The study of critical points of viability in elderly patients with visual impairment and the identification of the relationship between various mental or psychiatric diseases.
MATERIALS AND METHODS: A correlation analysis of key variables was carried out and the relationship between different variables was revealed. We conducted several tests to identify cognitive impairment, depression and learned about the quality of people’s sleep.
RESULTS: As a result of the study, 2,646 elderly people underwent 40.5% of men and 59.5% of women, whose average age was 69.69±6.22 years. More than half of the respondents had no physical activity and had a normal sleep quality. According to the study, psychological stability was positively associated with quality of life and negatively associated with depression and sleep quality (p <0.01). Depression, cognitive impairment, and sleep quality were positively associated with quality of life. Sleep quality showed a negative association with quality of life, depression (p <0.01) and cognitive impairment, and a positive association with depression (p <0.01).
CONCLUSIONS: Critical points of viability in the elderly are an important problem that requires special attention and care. Support from relatives, friends and specialists plays a special role, which can help prevent crisis situations and ensure that older people age with dignity. Preventive measures, proper nutrition, physical activity and regular medical examinations also play an important role in maintaining the vitality and quality of life of the elderly.

Maintaining long-term outcomes after rehabilitation in stroke patients: literature review
Abstract
The relevance of the stroke problem is determined by a significant increase in global morbidity and mortality, especially in developing regions of the world. Despite significant medical advances in recent decades, the burden of stroke continues to increase, posing a serious threat to the health of the planet’s population. Stroke is one of the leading causes of death and disability, having a significant impact on the quality of life of patients and the economy of countries. Over the past decades, a twofold increase in the number of cases of the disease has been noted, with the greatest number of fatalities and sufferers occurring in low- and middle-income countries. Separately, an alarming trend of increasing stroke rates among young population groups is emphasized, which indicates the need to expand preventive measures and improve the quality of specialized care. Russia also faces a high burden due to stroke: the annual number of new cases is increasing, and the mortality rate remains one of the highest among all diseases. The study confirms a significant difference in the incidence of strokes between men and women, reveals a high risk of early strokes in men and emphasizes the importance of taking into account the specifics of various population subgroups when developing a prevention and treatment strategy. International data confirm the trend of increasing strokes even in developed countries such as the United States and China, where there has been a significant increase in the number of fatalities and new cases. This circumstance requires the development of innovative approaches to the diagnosis, treatment and prevention of strokes, as well as intensified efforts to reduce lifestyle and environmental risks. Thus, the problem of combating stroke is of particular importance, since it affects almost all aspects of public health and has a significant impact on the development of national health systems.



Factors influencing the choice of rehabilitation measures for a disabled person with consequences of acute stroke
Abstract
Acute cerebrovascular accident (CVD) is accompanied by a wide range of long-term disabling functional disorders and disability, necessitating an individual approach to the formation of an optimal complex of rehabilitation measures for a disabled person with the consequences of CVD in order to make rational use of resources and improve the effectiveness of rehabilitation.
An analysis of the literature data allowed us to identify factors that complicate the rehabilitation of people with disabilities with the consequences of STROKE and determine their rehabilitation potential: the main one is cognitive status; auxiliary ones are age, level of motor disorders, aphasia (in particular, the ability to understand instructions), and psychoemotional state. These factors (pre-coded into the appropriate ICF categories) were used in the algorithm for assigning a disabled person with the effects of STROKE to a specific rehabilitation group to form a set of rehabilitation measures. Measurement tools for assessing the designated categories and criteria for assigning a disabled person to a rehabilitation group are proposed.
The developed algorithm makes it possible to formulate a realistic rehabilitation goal and form an adequate set of rehabilitation measures to achieve it, corresponding to the capabilities and abilities of a disabled person. It is assumed that people with disabilities who do not have cognitive impairments have a high rehabilitation potential and a favorable rehabilitation prognosis, which makes it possible to set an ambitious rehabilitation goal and use any available rehabilitation methods with a preference for active ones. The worse the cognitive function indicators, the lower the rehabilitation potential of the disabled person is assumed and the more realistic the goal is formulated; at the same time, preference is given to passive rehabilitation methods using methods adapted to the patient’s cognitive abilities and involving the disabled person’s relatives in the rehabilitation process.

To a practicing physician: formation of indications for the development of comprehensive rehabilitation measures for referral to medical and social expertise of citizens 18 years and older with HIV infection — AIDS
Abstract
When conducting an examination of citizens, carried out in the Federal State Institutions of Medical and Social Expertise (ITU), considerable attention is paid to the information provided by the medical organization (MO), including the main categories of disability in the “Referral for medical and social expertise by a medical organization” (form No. 088/y). Therefore, the correct and correct completion of the “Referral for medical and social examination by a medical organization” (form No. 088/y) is of particular importance. The article discusses the sections of form No. 088/y that require special attention of doctors of the Moscow Region, who draw up documents for referral of patients to the ITU, and which are the most important in relation to the development of IPR in citizens aged 18 years and older with HIV infection and AIDS.
