如何预防治疗Perthes病伴铰链式外展髋臼加盖成形术的植骨吸收或断裂——31例髋关接行改良Staheli术后中期预后良好

背景。髋臼加盖成形术覆盖髋关节并允许对Legg-Calve-Perthes病(LCPD)和铰链式外展进行髋部重塑。植骨吸收或断裂是一种需手术治疗的严重并发症。
目的本报告旨在评估改良Staheli术预防植骨吸收或断裂的效果。
材料与方法。对31例髋关节病例(29名患者)进行系列研究,手术时患者平均年龄8.1岁(6-14岁)。 主诉时间1至3年,平均1.52 ± 0.76年。评估髋关节各参数:Tönnis角、Sharp角、中心边缘角(CEA) 和髋臼覆盖率。对单侧病例,仅评估内侧关节间隙比和骺高比,
结果。术后随访平均时间47.8 ± 9.8个月,所有研究关节均为Catteral IV型,Salter-Thompson B型。7例处于关节碎裂期,24例处于再骨化期。根据外侧柱分类;只有2例被归为B/C型,29例被归为C型。最终随访发现内旋、外展、CEA和髋臼覆盖率显著增高。相反,Tönnis角和Sharp角明显减小。在单侧病例中,内侧关节间隙比和骺高比明显减小。截至最终随访,所有髋关节植骨均未出现再吸收或断裂。
结论。该改良Staheli术可预防植骨吸收或断裂。加盖术对伴LCPD和铰链式外展的畸形非球面头提供良好的髋臼覆盖,改善髋关节的临床和影像学结果。
