On the pathogenetic mechanisms of premature detachment of the normally located placenta

Abstract

The aim of this study was to study the systemic and regional mechanisms of endothelial dysfunction and thrombophilia in the pathogenesis of premature detachment of the normally located placenta.

To fulfill the assigned tasks, 340 women were examined. The main group included 72 women with premature detachment of the normally located placenta. The comparison groups consisted of 81 pregnant women in the III trimester of physiological pregnancy and 187 pregnant women in the III trimester with preeclampsia of varying severity.

Determination of viral load and DNA status of human papillomavirus type 16 by real-time PCR

Abstract

Integration of HPV DNA into the host genome is considered to be a key event in malignant transformation. This process results in disruption of El and E2 HPV genes, which is followed by up-regulation of HPV E6 and E7 oncogenes expression. It has been shown in this study that HPV type 16 is the most prevalent high-risk type in women with pathology of the cervix, with the frequency of HPV type 16 detection increasing along with the degree of neoplasia. With the use of real-time polymerase chain reaction the number of HPV type 16 DNA copies and its state were determined in clinical specimens. It was shown that the degree of the disease is associated with viral load and HPV physical state

Peculiarities of blood flow in uterus and ovarius in the presence of diffusive form adenomyosis

Abstract

The exploration of parameters of regional blood flow in aa. uterinae in 22 patients with diffusive form of adeno­myosis, in which ovulational menstruational cycle with the lack of lutein phase was registrated and 10 women of control group in dynamics of menstruational cycle. The levels of follicle stimulat­ing hormone, luteinizing hormone, prolactine, estradiole and pro­gesterone were examined in patients blood. The results showed, that adenomyosis is followed by normogonadotropic normoprolactinaemic insufficiency of ovaries, with the increased aa. uterinae vessel resistence background.

The frequency of detection and morbidity in children born to mothers infected with ureaplasmas

Abstract

The result of our investigations have shown that the determination of Ureaplasma urealyticum infection among pregnant women has recently increased. There is an enlargement of the number of women, who have got this infection after treatment. In this case more than 20% children have got Ureaplasma urealyticum and every fifth of them has clinical manifestation of intrauterine infection. But Ureaplasma urealyticum do not play important role in etiology of intrauterine infection. In most of cases CMV, Herpes I, II, Chlamydia trachomatis and Streptococcus В where etiological reason. May be Ureaplasma urealyticum call out pathological immune reactions in placenta and in the organism of a child. In its turn it stimulates active colonization of other pathogens. Presently the treatment of children who have Ureaplasma urealyticum is carried out only if there is clinical realization of infection. The question about the treatment of latent forms of Ureaplasma urealyticum infection needs farthest investigations of the influence of Ureaplasma urealyticum in the health of children of early age.

Donors of nitric oxide in the treatment of preeclampsia: influence on the state of the endothelium and hemodynamics in the functional mother-placenta-fetus system

Abstract

We investigated influence of nitrovasodilators on functional status of endothelium in preeclampsia with measuring of thrombomodulin, sICAM-1 and sVCAM-1 plasma levels. Changes in feto-placental and uteroplacental hemodynamics were analysed after treatment with isosorbide mononitrate and isosorbide dinitrate. Our results showed efficacy of nitric oxide donors in complex therapy of preeclampsia.

The properties of the microflora of the genital tract in women withendocervicitis

Abstract

The characteristics of the genital microflora of 70 non-pregnant women with non-gonococcal endocervicitis and 20 healthy women were characterized. In most cases of endocervicitis, a mixed infection was diagnosed, while chlamydia was most often detected in combination with herpes simplex viruses (HSV) and human papillomavirus (HPV). With monoinfection, ureaplasmas were more often found.

In 20% of healthy women, ureaplasmas were detected in cervical samples. It was found that in 83% of cases of endocervicitis, the vaginal microflora is involved in the pathological process, and most often this is manifested by bacterial and candidal vaginitis. The leading etiological role in the development of endocervicitis in non-pregnant women belongs to ureaplasmas, chlamydia, HSV and HPV.

Data of the own investigation to appraise the admissibility and efficiency of various RHT preparations aimed at correction of postovariectomic syndrome in patients with endometrial carcinoma are presented.

Progesterone-related changes in pro-inflammatory cytokines in recurrent miscarriage

Abstract

Habitual miscarriage (RMP) is a multifactorial, genetically determined disease. The NSP problem remains one of the most pressing problems in obstetrics and gynecology, as it entails not only a decrease in the birth rate, but also has a negative impact on the reproductive health of women. The frequency of spontaneous miscarriages ranges from 15 to 20% of desired pregnancies (Sidelnikova V.M., 2002). According to the Collegium of the Ministry of Health of the Russian Federation (2002), 50% of miscarriages occur in the first trimester, 25% of them occur in PNL. Moreover, every fifth desired pregnancy is interrupted spontaneously in the Russian Federation, which leads not only to medical but also demographic losses - 180,000 desired children are not born in the country. In the USA, habitual pregnancy loss (recurrent miscarriage) is diagnosed after three or more spontaneous abortions in a row and is observed in 1% of all married couples (Lee R.M., 2000).

Abstract

The article reviews the recent data (from 01.01.2002- 30.07.2004) concerning the role of HRT in prevention of postmenopausal bone loss and fractures. All the articles, that are included from Medline and Pub Med have been checked up in Cochran Controlled Register. The role of HRT remains great in treatment osteopenia. The effectiveness of HRT and depends on the length of duration, doses of estrogens, age of the patient.

I.I. Betskoy owe their appearance to the Orphanages in Moscow and St. Petersburg. The project for the construction of the Moscow Orphanage for 8000 children was drawn up by I.I. Betsky in 1763, planning a new educational institution for Russia, he set himself the main task - to save the children of low-income families and illegitimate children who "are born in extreme poverty, are abandoned by their parents and are betrayed to blind chance." In his note, signed on August 26, 1763, he asked the Empress to allocate a place in the center of Moscow, the so-called "Garnet Dvor", to build a house. Abstract

The article presents the analysis of structure and causes maternal mortality in North-Western region of Russia. The rate of maternal mortality is 8,9—62,7 per 100 000 lifebom infants. The main cors of maternal mortality in areas with high rate of maternal mortality are artifical abortion and bleeding. The ways of decreasing maternal mortality are discussed in this article.

Adenomyosis as an optional risk factor for uterine cancer

Abstract

The problem of adenomyosis malignization is controversial and insufficiently explored. Rate of the malignant transformation of adenomyosis varies from O.l to 24 per cent; as a rule, malignization of the stromal component of adenomyosis is described, works containing cancerous transformation description are isolated ones. Comparative clinical and morphological and immunohistochemical investigation of the cancerous transformation of adenomyosis (6 observations), endometrium cancer in combination with adenomyosis (16 observations), and typical adenomyosis (9 observations) is carried out. Obtained data indicate the precancerous optional significance of adenomyosis, risk of which increases in senior age groups. Endometrial adenocarcinoma does not exhibit a tendency to invasive growth into adenomyosis nidi; combined independent development of endometrium cancer and adenomyosis malignization with exo- and endophyte growth is possible, which determines the hypodiagnostics of malignizated adenomyosis. Morphological verification of malignizated adenomyosis demands comprehensive clinical and morphological investigation with broad study of operating material and with taking into consideration the stages of cancerous transformation morphogenesis established during the work. Late diagnostics and inadequate surgical treatment diagnose the unfavorable prognosis.

Rare complications of intrauterine contraception with formation of foreign bodies in the abdominal cavity and urine bladder

Abstract

2 rare cases of intrauterine contraception are presented. A 53-year old female patient had been having a Lippas intrauterine contraception device (IUCD) in the abdominal cavity for 19 years before it was occasionally revealed on cholecystectomy. The next female patient who had had a T-type IUCD for 6 years developed trophic changes of the uterine wall followed by the migration and penetration of IUCD into the wall and then the urine bladder cavity. Laparotomy and cystotomy with the evacuation of the foreign body and dissection of the walls of cystouterine fistular were performed. Both patients made a complete recovery.

Leonid Andreevich was born on February 11, 1933 in the Kirov region in a working class family. During the war, the family moved to Kirov, where he finished 7th grade. In the difficult post-war years, in 1948, he entered the Medical School (the former name of the Feldsher-Obstetric School) and graduated with honors in 1952.
