Therapy individualization of cardiac patients with acute coronary syndrome taking into account a phenotype of acetylation
Abstract
The paper presents research material on the individualization of treatment of cardiac patients. N-acetyltransferase activity in serum of patient with acute coronary syndrome may help to stratificate them into group according the severity of disease.

Safety of lipid-lowering pharmacotherapy in elderly patients with stable angina
Abstract
We estimated the safety of lipid-lowering pharmacotherapy in 248 elderly patients with stable angina. Rosuvastatin 10 mg/day with esetemib 10 mg/day was administrated to 81 pts of Group 1 with initial high LDL-level. Combination of atorvastatin 20 mg/day with esetemib 10 mg/day was administrated to 84 pts of Group 2 with moderate high LDL-level, while 83 pts of Group 3 with optimal LDL-level received simvastatin 40 mg/day. After 12 months of follow-up period we registered myalgia and/or myasthenia in 6 pts (2,4%), dyspepsia in 11 pts (4,4%) and increasing of transaminases in 9 pts (3,6%). In consideration of clinical significant events we interrupted the course of treatment in 4 pts (1,6%). The main causes among them were severe myalgia and/or myasthenia in 1 pt (0,4%), dyspepsia and gastrointestinal disorders in 3 pts (1,2%).

Influence of a short-term course of complex sanatorium treatment on the data of the patients with various vegetative dysfunctions of the cardiovascular system during a riverboat cruise
Abstract
The research subject was the influence of a 14-day course of complex sanatorium treatment on 48 patients with various vegetative disfunctions of the cardiovascular system (vagotonia, eutonia, sympathicotonia) during a riverboat cruise. The monitored groups showed registered trustworthy differences of cardiovascular system indices and the vegetative status measured both in the initial state and dynamically. The research revealed the existence of a correlation dependency of the Kerdo index and the dynamic segmentary diagnostics data. The discovered tendency to normalization of cardiovascular system indices and the vegetative status proved the efficiency of treatment of patients with vegetative disfunctions during a riverboat cruise.

Estimation of diagnostics resources of standard criteria of chronic cardiac insufficiency in patients with type 2 diabetes mellitus
Abstract
Prevalence of heart failure and diagnostics resources of standart criteries of heart failure was studied in out-patients with type 2 diabetes mellitus. Our results showed, that the main number of the patients with the type 2 diabetes mellitus have the heart failure. The object investigation increase revelation on 15-17%. Test 6MWD is the best index of it.

Influence of arterial hypertension on blood aggregation in geriatric patients with ischemic heart disease
Abstract
The study included 138 elderly men. The patients are divided into four groups: first group consisted of patients without clinical manifestations of the cardiovascular system; the second group includes persons with ischemic heart disease, stable angina of functional class II, chronic heart failure in stage I, I-II functional class; the third group - patients with ischemic heart disease, stable angina of functional class II, hypertension 1-3 degrees, congestive heart failure in stage I, I-II functional class. The control group included healthy men of mature age. Shown that elderly patients against coronary heart disease combined with hypertension there are changes in the state of aggregation of blood and found a close correlation relationship between the individual factors of blood aggregation. Only a multi-faceted impact on all indicators of blood aggregation in geriatric patients will create favorable conditions for improving the quality and longevity.

Molecular-genetic factors of hypertension in the workers of locomotive teams
Abstract
In the article there's a comparative estimation of influence of angionensinogen gene's polymorphism and 1st type receptors of angiotensin gene on the development of essential arterial hypertension with the workers of locomotive teams. In our investigation we included 112 workers of locomotive teams with essential arterial hypertension at the Arterial hypertension center of Samara railway station hospital based on the Russian railways. The material of investigation was the blood serum of the patients and genomic desoxyrhybonuclear acid. For the first time in the article it is considered the studying of genetic mutations among workers of locomotive teams. Within machinists and engine-drivers mates who have the high level stress profession, arterial hypertension becomes the professional disease. Genetic markers that were revealed serve as criteria of essential hypertension diagnostics and help purposefully to select the patients that need the primary prevention.

The analysis of efficiency of the remote control approach in prophylactics of mild bronchial asthma
Abstract
The majority of mild asthma cases and its low compliance level make investigators search for new instruments to improve asthma control and adherence, especially in this cohort. The goal was to research the new educational instrument of mild asthma management, its influence on the increasing of asthma control and compliance. 186 patients with mild asthma were enrolled in research that took 3 months. There were performed management instruments: personal asthma action plan, control visit with treatment correction and passing electronic educational asthma program. Changes in asthma control and compliance levels were revealed with ACT™ test (Asthma Control Test™), Compliance Index Assessment Questionnaire and in addition with knowledge control test results, immunologic parameters and etc. Educated in that way patients obtained higher asthma control, higher compliance level and were able to do self management finally. This instrument is required to be available to each patient with any form of asthma and should be further used in asthma management.

The clinical and laboratory characteristic of relapse in patients with HBeAg-negative chronic hepatitis B and high and low level of HBsAg
Abstract
This article discusses the results of complex examination of relapse in patients with HBeAg-negative chronic hepatitis B option depending on the level of HBsAg. The data showed more pronounced changes in Basic biochemical and immunological parameters in patients with high levels of HBsAg. Shows that the level of HBsAg in patients with chronic hepatitis B correlates with the level of serum HBV-DNA and alaninaminotransferazy.

Projective psychodiagnostics of factors of a drug addiction
Abstract
At present drug addiction is a real threat for the people's health in our country. According the data of the federal statistics observation in 2009 there were 555 thousands of registered drug users, the number of drug addicted is 358 thousands, 87% of them take heroin. That is the reason why a more successful rehabilitation program needs to be created. To make such, we need to narrowly examine the psychological factors of drug addiction, also by means of projective methods, find out what are this factors and how do they influence the possibilities and capabilities of the patients to recreate.

Dynamics of psychic impairments before and after carotid endarterectomy
Abstract
In article examine the structure and dynamics of psychic impairment at 103 patients before and after carotid endarterectomy is considered, including catamnesis supervision in 3 and 6 months after surgical treatment. The analysis of results has shown considerable positive dynamics of psychic impairment as a result of carotid endarterectomy (improvement at 76,7 % of patients).

The structural and functional state of the membranes of erythrocytes in patients with COPD and markederythrocytosis
Abstract
In 74 patients with COPD stage II with erythrocytosis in acute investigated the structural and functional state of the membranes of erythrocytes. Physico-chemical characteristics of erythrocyte membranes studied by determining the content of cholesterol (LDL), total phospholipids (PL) and their ratio (CH / PL), as well as the method of fluorescent probes. Established microviscosity (J286) lipids, calculated the degree of clustering (fragmentation) of the structural proteins (Jkl). To assess the intensity of free radical oxidation (FRO) membranes of red blood cells determined malondialdehyde (MDA) - a secondary stable product FRO. Antioxidant components of oxidative stress (OS) in the membranes of erythrocytes was determined by the level of vitamin E. Established that patients with COPD there is increased compared with healthy people and matrix microviscosity of membrane lipids of erythrocytes, as well as elevated levels of clustering (fragmentation) structural proteins. These phenomena are expressed at significantly greater in patients with significant erythrocytosis and is closely connected with an increased ratio of cholesterol / PL, as well as with higher levels of activity of free radical oxidation of lipid-protein structure of erythrocyte membranes. It can be assumed that the identified changes are the basis of disturbed rheological properties and transport functions of erythrocytes.

Teratogens influence of antiepileptic drugs: the study of the problem in the light of common population investigations of congenital disorders of development
Abstract
Detection of teratogens is the main objective of population monitoring. Risk of major malformations in general population of Samara region was 2,6%. We evaluated outcomes of 280 pregnancies of women with epilepsy. Level of major malformations , including due to genetic and chromosomal disorders , accounted for 2,9 %. We found no clear relationship between the frequency of major malformations, and taking his mother antiepileptic drugs.

Correlation mathematics models of clinical picture peculiarities of post-hysterectomy syndrome
Abstract
For the last few years the statistic data all over the world demonstrate a significant increase in the incidence of gynecological disorders requiring surgical treatment. The most common method of treatment is hysterectomy with or without the uterine appendage removal. In the course of time most of the patients develop post-hysterectomy syndrome of different severity, due to the lack of genital steroids (estrogens in particular), that demands medicamental correction. We carried out the investigation aiming at revealing the peculiarities in the clinical picture of this syndrome in the patients who have undergone surgery.

Estimation of phenotypical signs in women with connective tissue dismorphism
Abstract
In this paper the phenotypical women with the signs of connective tissue displaceability were intercommunicate. Anamnesis date were investigated between 500 women of reproductive age, somatic profile state and the signs of connective tissue displaceability were assessed according to S.N. Buyanova and coauthors' scale. Determination method was used to analyze the received data. The results of the research showed that asthenia, post gravidarum striae, greater brittleness of nails, frequent headaches, osteochondrous, eyesight disorders, flat foot, varicosity and vasoneurosis refer to the most common phenotypical signs. Correlation dependence of obstetrical gynecological pathologies are less important. But we should pay attention to this problem when we deal with prediction development of gynecological disorders.

Abstract
As a result the taken system estimate of men and women health who leave in Samara city and its region at the age of 18 till 25 shows the main factors in the structure of men and women reproduction health at the contemporary stage. This factors are genital infections, somatical health and social status. Reproduction health of a young family at the contemporary stage depends on the occasional extract of its partners, which brings reproduction potential of married couple down.

Morphological and morfometrical criteria of postabortion andometris development
Abstract
The aim of research: to reveal the criteria of development of postabortion andometris taking clinical morphological parallels into account.Research material: 100 women with clinical signs of the complicated abortions and 80 women with spontaneous abortions without complications; andometris analyses at 180 women with spontaneous abortions in term till 22 weeks of pregnancy and at 10 women at medical interruption of pregnancy.Results of research: the most significant pathogenetic risk factors of development of andometris after spontaneous abortions is promiscuity (20 %), extragenital pathology, in particular diseases urogenitalis systems (44 %), use of intrauterine contraception (21 %), and also a gynecologic pathology: nonspecific (87 %) and specific (55 %) inflammatory diseases of female genitals, infringement of a menstrual cycle (18 %).The conclusion: for decrease in frequency of development of complications after spontaneous abortions and improvement of quality of individual treatment-and-prophylactic rehabilitation actions it is necessary to consider morphological and morfomatrical criteria of development of andometris.

Efficiency of no-hormonal methods of premenopause disorders correction
Abstract
142 women were examined in premenopause with dissimilar evidence of climacteric syndrome in which the leading role was played by psychoemotional and neurovisceral dissociations. Among them three groups are singled out: the first group includes 57 women who took medication from selective serotonin reuptake inhibitor (SSRI) group, the second group includes 55 women who took phitoestrogens and physical therapy, and the third group of comparison includes 30 female patients. The issues of undertaken study showed a great efficiency in reserving psychoemotional dissociations with SSRI group medication than neurovisceral. The phitoestrogens in common with physical therapy efficiently reserved psychoemotional and neurovisceral dissociations.

Efficiency of the system of early prevention of a perinatal pathology: the health state of pregnant women and newborn children
Abstract
The analysis of efficiency of the new scientifically-based system of early prevention of a perinatal pathology in Samara health care system on the basis of studying of health state of pregnant women and newborn children for 1997-2008 is carried out. Correctness of the chosen strategy of protection of population health is proved by an unidirectional tendency of decrease in perinatal and infantile death rate and disease of newborns.

Features of activities of life quality and level of desease control in pre-school children with bronchial asthma
Abstract
The aim of treatment of BA is to control the disease and improve the life quality of the
patients. 112 patients were examined to estimate the control over the disease and life quality
of preschool children suffering from BA. For life quality estimation Childhood asthma
questionnare for 4-7 year old children was used. BA course control in children was estimated
with the use of the questionnare Test for Asthma Control in Children. Uncontrolled course
of the disease was revealed in 80% of patients. This group of children had a lower life quality
level in comparison with those with controlled course.

Psychological status and course dynamics of functional dyspepsia in schoolchildren according to prospective data
Abstract
In article the data about psychological features of children of school age with a functional dyspepsia is cited. High prevalence of uneasiness and internal intensity is shown. Influence family and social factors on formation FD, in particular adverse conditions of residing, low material security and, especially, disharmonious family relations (90,1 % of cases) is taped. As results of the spent symptomatic and complex therapy including psychological correction were estimated. At the schoolboys receiving complex treatment in 2 times is more often full cupping of symptoms of disease became perceptible and in 2 times resistance to spent therapy was less often registered, positive dynamics of psychological changes became perceptible. At children by whom symptomatic therapy was spent, internal intensity, uneasiness, isolation during research accrued.

Efficiency estimation of basic preparations used in the treatment of children and teenagers with different types of juvenile idiopathic arthritis
Abstract
For specification of diagnosis JIA and definition of the importance of pathogenetic changes research of parameters cellular and гуморального immunity, the most informative is shown at the given pathology, namely: CD4 +, CD4 +/CD8 +, CD8 +, CD16 +, CD95 + in whey of blood, and also a spectrum cytokines: TNF, IL6, IL1, IL4. The optimum choice of a basic preparation for treatment is carried out for concrete form JIA. At the polyarticulary form with a high degree of activity of inflammatory process the preference is given ciclosporin and methotrexate. At JIA activity variant oligoarthritis preparations of a choice are methotrexate, ciclosporine, auranofin, at nonactivity variant oligoarthritis - auranofin, sulfasalasin. However it is necessary to note, that any of basic preparations is not exclusive at an individual choice of treatment, and very much frequently it is necessary to select empirically during treatment this or that preparation.

Early arthritises in children and teenagers: diagnostics and prospects
Abstract
High blood levels of CD4, CD8, CD16, CD95. Pand anti- inflammatory interleukins were revealed at active stage of JIA particulary in pts with polyarthritis and extended olygoarthritis. Changes of mean lymphocyte morphometric measures linearly inversely correlated with relative lymphocyte markers level what proves relationship of processes of proliferation? Cytotoxicity and elevation of apoptotic cell count in blood. Howerver increase of programmed death cells may reflect not only proliferation but also capability of cells to induce cell death program in presence of provocative factors. In pts with very high humoral level of lymphocyte markers and cytokines appropriate therapy more often induces clinico-laboratory remission than in pts with lower values. Immune and cytokine status in children and adolescents with JIA determines evolution of arthritis. Early administration of disease modifying drugs more often induces of clinico-laboratory remission in pts with high levels of lymphocyte markers and antiinflammatory cytokines.

Comparative analysis of cytokines in children with chronic virus hepatitis В and C
Abstract
In clause the immune factors influencing on the pathogenesis of a chronic virus hepatites B and C at children are considered. For the characteristic of the immune answer at patients with CVH B and CVH С in the replicative and integration phases we study a sufficient spectrum cytokines. Researches of whey of blood of children sick of a chronic virus hepatites В and h in replicative and integration of a phase on a parity of a level cytokines were carried out. Interrelations cytokines with cytolysis at children with a chronic virus hepatites В and have been considered С.

Health status of the children with exogenic-constitutional obesity
Abstract
We have made retrospective analysis of 138 case histories of children with exogenic-constitutional obesity staying in endocrinology department of State Children Clinic №1 in Samara from 2000 to 2009, 92 of them are boys and 46 are girls at the age from 3 till 17. It is revealed that predisposition of male sex, preschool and young school ages, living in big urbanistic centers are important points in appearing of examined pathology. High mass index and high length in birth are peculiar to obesity development. Hereditary predisposition in obesity, diabetes mellitus, hypertonic and ischemic heart disease is also influence on health condition in young patients. Insufficient attention to the problem of obesity in children and teenagers in Samara region makes it necessary to provide more qualified diagnose by means of medical stuff and to introduce better preventive measures among parents and their children.

Complex treatment of wounds and wounded infections of a skin and soft fabrics by application wounds coverings
Abstract
In article questions of local treatment of wounds and wounded infections of a skin and soft fabrics by application wounds coverings of "Aktivteks-Butol" in experiment, carrying out of microbiological researches are considered. Advantages of the given kind of local treatment of wounds over others on 36 laboratory animals are considered. Supervision at restoration of defects of an integument at laboratory animals are in detail described at application wounds coverings of "Aktivteks-Butol", in comparison with traditional therapy («Levomekol» ointment, solutions antiseptics). The conducted researches confirm that application wounds coverings with the combined antibacterial component in complex treatment of wounds and wounds infections of a skin and soft fabrics in comparison with traditional therapy allows to reduce terms of healing of a wound and to create favorable conditions for regenerist processes in a wound.

The results of an application of the new approach to treatment of patients with crises of proximal department of the humeral bone
Abstract
Treatment of patients with crises of proximal department of a humeral bone is an important problem of modern traumatology. In our work we have carried out the analysis of treatment of 150 patients with the given trauma which have been divided into three clinical groups, depending on a way of medical intervention. Application of the new approach to treatment of patients with crises of proximal department of the humeral bone including use of a way of surgical treatment of given patients, the tool for performance of the most responsible stage of operation and devices for carrying out of the regenerative period, has allowed to lower number of failures from 77,8 % to 24,3 %.

Malfunction peculiarities of the hemostasis system in patients with severe acute pancreatitis
Abstract
Blood coagulation system plays an important role during the severe acute pancreatitis. With the purpose of delimitation of the pathological process in the pancreas, a process enhanced fibrin, which leads to abnormal strengthening of hypercoagulability, which in turn impedes access of drugs to target organs. As the disease progresses and the depletion of the organism, the stage becomes hypocoagulation hypercoagulability, manifested hemorrhage and bleeding. Early diagnosis and treatment of DIC can significantly improve outcomes and reduce the number of deaths.

Stimulation of neoangiogenesis in patients with chronic ischemic lower extremities by autological progenitor bone morrow cells
Abstract
The results of neoangiogenesis has been studied for the treatment of 42 patients with arterial atherosclerosis occlusive disease of lower extremities who are not candidates for treatment surgical. We studied the results in the 3 groupies including 14 patients in the each. Autological progenitor cells (CD 133+) were transplanted in I-st group. Leucocytes bone morrow cells (CD 34+) were transplanted in II group. The patients of III group was injected isotonic solution as a placebo. All of solutions were injected in internal and external muscles of shin. We appreciated our clinical results in Rutherford's scale. In conclusion, we observed what the improvement of clinical condition in patients (with a transplantation of CD 133+ and CD 34+ of I and II groups) was trustworthy by Mann-Whitney's criterion.

Analysis of using the training with electromyographic biofeedback in patients with the cervical spinal cord injury has been presented in the article. Biomechanical method and functional scale have been applied for estimation of efficiency. Application of biofeedback promotes to formation of more right biomechanical and innervation of grip structure: pressure distribution among various hand parts during force (cylindrical) grip «сurves» to norm ore redistribution between parts of hand. Self-service, handicap of patients are improved.

Pathohistological characteristic of urinary bladder malignant growth
Abstract
This article presents the detailed morphological structure of urinary bladder neoplasms of 628 patients who have undergone surgical treatment in urological hospitals of Republic Mordovia. The analysed histological material in 76 % cases showed malignant tumors. The diagnostics indicates that incorrectly executed biopsy in 38 % gives a false negative result and subsequently it confirms by incisional biopsy. In 12 % of cases when urinary bladder transurethral resection is executed the presence of malignant growth also doesn't confirm.

Comparative analysis of the efficiency and safety of surgical treatment methods of inner atherosclerotic carotid stenoses in groups of patients with high surgical risk
Abstract
There has been conducted comparative analysis of the results of operations of carotid edarterectomy (CEE) and carotid angioplasty with stenting (CAS) in groups of patients at high surgical risk with atherosclerotic carotid stenoses. The groups under research included patients who were operated on their carotid arteries in the period 2007-2010. The obtained results of treatment of stenosing affections of the carotid arteries testify to the safety and high effectivity of carotid angioplasty with stenting.

Modern approach to the wounds sealing of the lung using videothoracoscopy
Abstract
The aim of the work was to improve the results of pulmonary resection by developing and implementing ways of sealing the wounds of biological adhesives, and drugs. Were analyzed the results of applying additional methods sealing line of manual and mechanical suture after atypical resection of the lung/.Methods of aerial and hemostasis are particularly necessary for emphysematous changes in pulmonary tissue. Application of differential approach to the choice of sealing the seam line to reduce the number of complications in the postoperative period.

Planning and caring out orthopedic treatment using dental implants in patients with generalized pathological dental abrasion with decreasing occlusional height and partial toothlessness
Abstract
The individual approach is necessary for decision-making concerning a way of prosthetics on dental implants to each patient. As shows clinical experience, a main role at planning of implantation from the orthopedic point of view play condition of maxillotemporal joint, condition of the teeth and teeth rows, occlusion. Pathological dental abrasion ia a heavy and widespreading disease. Partial abcense of teeth complicats this clinical situation. On the basis of the anamnesis data, complaints of the patients, clinico-tool survey and radiodiagnosis established two forms of disease: pathological dental abrasion with poorexpressed signs of decreasing occlusional height and with considerably expressed signs of decreasing occlusional height. On this basis the differentiated approach to treatment of the given category of patients is offered. The algorithm of treatment is developed.

Clinical picture and methods of treatment of patients with TMJ osteoarthrosis due to a chronic microtrauma
Abstract
There are various clinical symptoms and methods of complex treatment of TMJ osteoarthrosis, caused by a chronic microtrauma. Within the research project, 52 patients aged from 41 to 64 years old have been diagnosted with TMJ osteoarthrosis, on the basis of electromyography, a computer tomography, registration of movements of the mandibula, results. The results of the research work have allowed to develop medical treatment plan directed to restore the height of the lower part of the face, normalisation of occlusial relations and elements of TMJ, the ligament-muscular system.

The importance of computer tomography in diagnostics of dental root perforation
Abstract
There is an increasing tendency for computer tomography to be in a strong position in diagnostics of stomatological nosological forms. In this article the comparative informative analysis of intraoral dental roentgenography, orthopanthomography and dental CT in diagnostics of root extraction in the same patients is presented. Based on our findings high efficiency of dental CT in comparison with other methods of beam diagnostics is shown.

Social adaptation and the problem of self-estimation change in patients with defect of an auricle (social-psychological aspect)
Abstract
For the purpose of the permission of questions on necessity of operative treatment and rehabilitation including psychological, research with group of patients (58 persons), having in the anamnesis the congenital or got defect of an auricle, on the basis of change of two basic aspects of functioning of the patient has been conducted: personal and social. Investigating interpersonal mutual relations in collective and in a family, it has been revealed, what most unadapted in the social plan is groups of patients with subtotal and total defects of auricles of the first group (got), by us it has been revealed, what most unadapted in the social plan the group of patients with posttraumatic: criminal and burn deformations.

Role of Samara Regional Center of prophylaxis and treatment of atherosclerosis and dyslipidemy in the organisation of medical aid to elderly patients
Abstract
We presented data about role of Samara Regional Center of prophylaxis and treatment of atherosclerosis and dyslipidemy in the organisation of medical aid to elderly patients with risk factors and manifest atherosclerotic diseases. Systemic and integrated approach to diagnostics and treatment of dyslipidemy along with primary and secondary prevention of cardiovascular diseases in geriatric patients was realized and theoretically substantiated.
We created medical and preventive module for treatment of patients with CHD, early diagnostics of atherosclerosis in elderly patients with risk factors and their stratification to homogenous risk categories. It is necessary for lipid-lowering pharmacotherapy strategy selection, estimation of target level of blood atherogenic lipids, statin needs, dose escalation and required multidrug regimen.

Dynamics analysis of the sizes of average wages of local medical nurses and nurses in other medical institutions in Samara
Abstract
The spent comparison of the sizes of average wages of local medical sisters of city district Samara in nominal expression for the investigated periods shows its growth. Use as a tally of size of a living wage allows to reveal only small increase in the relative size of wages. Thus, use of new system of a payment by heads of medical institutions in city district Samara has led to increase of the size of wages of local medical sisters even in nominal expression of a part of establishments, and growth of real purchasing capacity of the paid wages taking into account an index-defljatora was negative in all establishments, however the range of degree of decrease was considerable enough.
The carried out analysis of efficiency of new system of a payment in public health services in system of municipal public health services an city district Samara was shown by absence of positive changes in increase of level of a payment in real expression of local medical sisters and decrease in differentiation of work of workers of the higher and lowest qualification in comparison with the Uniform scale of charges.

Reproductive health of population and its influence on demographic situation
Abstract
The aim of research: to optimize the organization of medical care to the fertile women. Research material: 268 women with clinical signs of the complicated abortions and 268 women with spontaneous abortions without complications; analysis of andometris at 180 women with spontaneous abortions in term till 22 weeks of pregnancy and at 10 women at medical interruption of pregnancy. The conclusion: efficiency of treatment-and-prophylactic actions with spontaneous abortions is based by the patient on a timely and competent estimation of conditions of reproductive system depending on multifactorial genesis pregnancy interruptions.
