Abstract
The article is devoted to functional activity of platelets in geriatric patients with coronary heart disease. It is shown that patients of advanced age represent the most vulnerable group of risk of development of cardiovascular complications.

Mean platelets volume as an adverse prognostic marker in geriatric patients with coronary heart disease complicated by atrial fibrillation
Abstract
The article presents in comparative perspective the number of platelets and their volume, lipid metabolism, vascular-platelet hemostasis in elderly people without clinical and instrumental manifestations of cardiovascular disease, in patients suffering from coronary heart disease and in the group of patients with ischemic heart disease complicated by persistent forms of atrial fibrillation. Correlation analysis between the studied indicators was conducted. It shows the relationship between the risk of atrial fibrillation and laboratory parameters in geriatric patients with ischemic heart disease.

The efficacy and safety of thrombolytic therapy of acute coronary syndrome with ST-segment elevation in pre-hospital stage
Abstract
The article provides an analysis of the results of thrombolytic therapy in 85 patients with acute coronary syndrome with ST-segment elevation. The patients were divided into groups according to the time from the onset of pain to the conduction of thrombolysis: group 1 CLT up to 1 hour, group 2 with CLT from 1 to 2 hours, group 3 with CLT more than 2 hours. It is shown that the pre-hospital thrombolysis with the use of fibrin-specific plasminogen activator conducted within 1 hour from the onset of the symptoms of the disease is the most effective and safe. When you increase the time before treatment, the effectiveness is reduced.

Characteristics of microcirculation in veterans of war with arterial hypertension
Abstract
The article presents the results of a survey of the functional condition of the microvasculature in combat veterans with arterial hypertension by laser doppler flowmetry. We investigated the condition of basal blood flow, as well as the change of microcirculation affected by occlusion test. The obtained results show significant microcirculatory disturbances in combat veterans suffering from hypertension. Represented data indicate the predominance of passive mechanisms of microcirculation regulation, low adaptive capacity and reserve capabilities of microvasculature in the studied group.

This research investigates the central hemodynamic parameters in elderly people, depending on their physical fitness after a brief physical activity with high proprioceptive activation. It reveals the functioning of the central mechanisms of somatic and autonomic interaction in elderly athletes and functional failure in physically untrained people.

Bacteriological characteristics of pathogens of community-acquired pneumonia in hospitalaized patients
Abstract
The study was made on 98 patients. The structure of etiologically significant pathogens of CAP in adults of different ages is studied. Much attention is paid to microbiological test method in determining the etiology of pneumonia. For the first time in Samara region we presented the data on the study of urine antigen pneumococcal capsule in-vitro immunochromatographic test Binax NOW® Streptococcus pneumoniae.

Peculiarities of pregravid preparation of women with non-differentiated connective tissue dysplasia
Abstract
The analysis of risk factors, case history peculiarities, defining the role of magnesium ions in the metabolism of connective tissue and formation of dysmorphism, as well as revealed correlations enabled us to develop therapeutic and prophylactic measures for pregestational training for women with connective tissue dysplasia. Designed therapeutic and preventive complex for women with connective tissue dysplasia allows to adjust revealed dysplastic disorders, prevent their progression and perform prophylaxis of gestational complications and connective tissue dysplasia in the fetus.

Complex assessment of somatic health and psychophysical development of children with perinatal hiv infection contact brought up in closed institutions
Abstract
The paper presents the results of a survey of 68 children aged from 0 to 18 months with perinatal HIV infection contact brought up in a closed institution. We carried out a complex analysis of health and social-hygienic characteristics of HIV-infected mothers. We revealed high morbidity rates and physical and psychological abnormalities of all pupils in specialized orphanage for infants born by HIV-positive women.

Comparative analysis of psychological development of children brought up in infant orphanages and in foster families
Abstract
Children growing up in infant orphanages are the least socially protected group. Children, who are brought up in orphanages, develop severe deformation of personality, identity and intellectual development. Deprivation of maternal care leads to the delay in a child’s development - physical, intellectual and social - and may manifest as symptoms of mental and physical diseases. The lack of maternal care, permanent residence in a closed team determine the level of mental development of these children. In this paper, a comparative analysis of neuropsychological development in pupils of children’s homes and children in a foster family.

Prevalence of anemia in patients of pediatric hospital
Abstract
The article presents the results of a retrospective analysis of medical records of patients treated at the Children’s city hospital №1 in Samara. We carried out a complex analysis of the general blood test and iron metabolism in order to identify the prevalence of anemia. We revealed high prevalence of iron deficiency anemia in a group of children aged 0-3 years, and we also found a group of children with latent iron deficiency.

Comparative analysis of methods of diagnosis of chronic fetal hypoxia using clinical epidemiology tests
Abstract
The purpose of the study was to estimate the diagnostic value of different methods of diagnosis of chronic fetal hypoxia, to select the most informative ones and to optimize obstetric tactics. In a randomized prospective study chronic fetal hypoxia was revealed in 216 pregnant women in the group of high risk of developing severe placental insufficiency using diagnostic tests and taking into account the fetal daily rhythmostasis. Retrospectively, we used diagnostic tests of clinical epidemiology to calculate sensitivity, specificity, positive and negative predictive values of the results, the diagnostic accuracy of methods for diagnosing chronic fetal hypoxia, taking into consideration the histological verification of the diagnosis of placental insufficiency and the blood pH determination in the umbilical artery (blood was obtained before the first breath). The study showed that a complex approach to the diagnosis of chronic fetal hypoxia with placental insufficiency, taking into account the fetal daily rhythmostasis, has the greatest diagnostic value in comparison with the other methods.

Clinical effectiveness of application of the matrix system to restore the contact region side teeth groups
Abstract
In the course of a randomized, open, comparative, controlled, parallel-group study evaluated the clinical efficacy of matrix systems to restore contact areas of the teeth of the side group. The study involved 33 patients with defects of hard dental tissues (2 class on Black). Randomly selecting created two groups: 15 patients in the control group, treatment which includes recovery by the retaining ring sectional matrix and the wedge, as well as the study group, 18 patients, instead of fixing ring which set the locking system of the nickel-titanium alloy adaptive worktop. Qualitative evaluation of interventions made in 5 days after the restoration of the proximal regions of the teeth of the side group.

Use of navigation system during positioning of dental implans
Abstract
This article raises the problem of positioning of dental implants in the upper and lower jaws. Aim of this study was to evaluate the experiment results on the phantom model of implant placement using dental navigation system. As a result of studies on implant placement using dental navigation system on a phantom containing the elastic tissues of the face, the maximum angular deviations between the axes of the implant in the sagittal plane were 10 37I, and in the frontal plane 20 16I.

The article presents the results of medical and sociological research of dentists’ attitude to organization of dental aid to elderly people in Samara region. The findings established the positive attitude of respondents to possibility of providing dental care to elderly people in the form of specialized dental department for this age group patients. The use of dental haemostatic set «Haemoblock DENT» in the extraction of teeth in patients with idiopathic thrombocytopenic purpura
Abstract
We have estimated the efficiency of dental haemostatic kit «Haemoblock DENT» application in planned tooth extraction on patients with idiopathic thrombocytopenic purpura. Comparative analysis of results of treatment in immediate postoperative period of two clinical groups with idiopathic thrombocytopenic purpura has been performed according to the evidentiary medicine principles. Reasonably improved clinical outcomes, such as considerable socket bleeding time decreasing, postoperative alveolitis risk reduction and early masticatory apparatus function recovery have been achieved in all patients of the clinical group where dental haemostatic kit «Haemoblock DENT» has been applied. This result permits to recommend it for clinical use.

Currently, one of the most pressing problems in orthopedic dentistry is the complete absence of teeth. The aim of the study is to optimize the prosthetic patients with complete absence of teeth on the lower jaw by clinical and mathematical study of the distribution of masticatory load on the prosthetic bed. The study was performed on 19 patients with complete absence of teeth in the lower jaw.

Justification of effectiveness of clinical application of demineralized spongiosa «LIOPLAST»® during surgical resection of the apex of the tooth root
Abstract
In the practice of operative dentistry, surgery resection of the apex of the tooth root in the treatment of chronic granulomatous periodontitis does not lose its relevance. To prevent possible complications during the operation of cystectomy, osteoplastic materials are applied. According to the research studies, demineralized spongiosa «LIOPLAST»® reduces the possibility of clinical symptoms (swelling, pain, fever) after surgery resection of the apex of the tooth root, and also contributes to the creation of favorable conditions for regeneration of bone tissue.

Modern approaches to treatment of purulo-necrotic lesions combined with diabetes

Diabetes is a socially significant problem in the modern society. Complications of this disease suppose a comprehensive approach to their treatment. This article presents some aspects of the pathogenetic treatment of purulo-necrotic forms of diabetic foot. Specifically, it dwells upon a set of basic therapy in combination with the treatment of wounds by a controlled vacuum therapy using fibrinolytic, antithrombotic, vasoprotective agents and veinotonic. Reconstructive surgery in treatment of patients with malignant tumors of the pelvic ring
Abstract
22 patients with malignant tumors of the pelvic ring were observed. 9 (41%) patients underwent reconstructive surgery with endoprosthesis replacement, 5 (23%) - with iliosacral or ischiosacral fixation, 4 (18%) - with combination of steel nails and PMMK, and 4 (18%) - with PMMK only. Results: wide resection was achieved in 82% cases. 4 patients had disease progression within 8-16 months. In all patients functional results after surgery were perfect or good. Complications were observed in 8 (36%) cases.

Results of cytoreductive treatment of patients with synchronous bilobar liver metastases of colorectal cancer
Abstract
The program of study includes 115 patients diagnosed with colorectal cancer, stage 4, with synchronous single bilobar liver metastases. In 61 patients (control group) cytoreductive surgery was carried out with atypical liver resection. In 54 patients (study group) cytoreductive tumor removal was followed by RFA of liver metastasis. RFA bilobar liver metastases allowed to increase significantly the number of cytoreductive operation R0, reduce the amount of intraoperative blood loss, risk of postoperative complications and, consequently, reduce the time of patient’s staying in hospital. RFA improved long-term outcomes, increasing survival rates and quality of life of patients on a FACT-C scale.

Fnaesthsia in oncological patients with mediastinal compression

Most common complication of massive mediastinal tumors are tracheal and bronchial obstruction and superior vena cava compression. Early in the disease there may be no symptoms, compression may present with life-threatening manifestation during general anaesthesia. Variability indicators of noninvasive hemodynamics with extensive oncologic operations under the combined anesthesia with the use of sevoflurane and desflurane
Abstract
The leading principles of noninvasive monitoring of hemodynamics are accuracy, reliability, possibility of dynamic supervision over the patient, complexity, existence of the minimum quantity of complications, practicality, low cost, availability of received information. Parameters of noninvasive systemic hemodynamics, which can be determined by the device NICO, play an essential role in the assessment of the condition of a patient when carrying out an anesthetic support during extensive oncologic operations. Intraoperatively we observed variability of cardiac index, cardiac output, and stroke volume depending on used anesthetic. Noninvasive monitoring of hemodynamics is informative and traces indicators of systemic hemodynamics at any surgical manipulations, and serves as a reference point during anesthesia. With application of Desflurane, fast awakening of the patient with smaller influence on hemodynamics in comparison with Sevoflurane is observed.

Preliminary results of the new cementing method of the acetabular component of the coxofemoral joint arthroplasty

The analysis of the activities polyclinic in conditions modernization of health care
Abstract
The article is devoted to health care services in primary care. The relevance of the study determine the conductivity of activities aimed at improving the availability and quality of care. The article analyzes the security of the population by medical personnel, studied the dynamics of the volume of outpatient care. We have shown that despite increased preventive orientation, significant changes in performance on the part of the total mortality, hospitalization in hospitals did not happen.

Peculiarities of delivering medical aid to patients with acute coronary syndrome in Samara region by the example of clinical statistical groups in Samara Regional Clinical Cardiologic Dispensary
Abstract
This article includes the features of delivering medical aid to patients with acute coronary syndrome in the territory of Samara region. In this article 3 approaches to formation of clinical statistical groups are described and a calculation of cost of treatment of one patient with ST-elevated myocardial infarction is made. It was proved that application of an early reperfusion in the form of stenting of coronary arteries has considerable advantages compared to thrombolysis and conservative treatment.

We studied clinical and functional changes in the body of schoolchildren living in the vicinity of the major highways and schoolchildren living in an area remote from major highways. We identified clinical and functional peculiarities of the organ of vision in pupils living in the vicinity of the major highways, contrasted to the control group of students. That reveals the necessity of preventive measures aimed at inclusion of functional methods, which detect changes of the vision, and introduction of antihypoxic therapy to prevent abnormalities on the preclinical stage.

The intraocular pressure at carrying out accommodative load according to the presence of optical aberrations
Abstract
The relationship of hydrodynamic processes with accommodation receives more and more evidence in the works of various authors. For the first time the influence of accommodative load on the level of IOP in eyes with different values of astigmatism was revealed in this research work. The mean value of intraocular pressure reduction was the greatest in patients with astigmatism up to 0.75 D. Patients with astigmatism up to 0.75 D showed a statistically significant increase in the level of intraocular pressure after accommodative load compared to the initial data.

Special features of standardization of new raw material - dandelion herb (Taraxacum officinale Wigg)
Abstract
By the use of microscopy, thin-layer chromatography and spectroscopy the quality indicators of dandelion herb (Taraxacum officinale Wigg.) were developed. Some distinctive anatomical and morphological signs of raw material were defined. Parameters of qualitative analysis were defined by using thin-layer chromatography and spectroscopy. The content of total amount of flavonoids in air-dry material should be at least 1.5%.

Microscopic and phytochemical studies of the underground organs of bloodroot (Potentilla recta L.)

An anatomical and morphological study of the Potentilla recta L. roots was done to reveal characteristic diagnostic features. The optimal conditions for the extraction of raw materials were defined. A qualitative analysis of the studied plants was done with the use of thin-layer chromatography and spectrophotometry.
