例健康儿童桡骨萎缩性骨折不愈合伴有严重骨质溶解:手术成功。患儿腓骨同种异体移植和自体移植生长因子治疗

背景:前臂骨折是儿童和青少年中最常见的骨折,男性多于女性。在过去的20年中,外科手术指征的增加导致了并发症的增多,其中骨折不愈合在儿童中极为罕见和严重,其发病率已增加。我们报道了1例既往健康的4岁儿童被误诊为前臂萎缩性骨折不愈合伴有严重骨溶解,之后我们使用腓骨同种异体移植物和自体生长因子成功治疗的方法。
临床病例:1例四岁男孩因严重骨质流失和骨活检Gorham-Stout综合症阴性被收入我院,患肢同时出现反应性骨组织伴有血管异常、坏死性骨软骨碎片和巨大的单核细胞。其他实验室检测未显示任何改变,因此排除了所有引起儿科骨溶解的原因。患肢骨折已接受了几次效果不佳的手术,在我们施行了具有髓质生长因子的自体腓骨移植并用Kirschner钢针固定手术后获得了良好预期。在28个月后的随访中,该患儿患肢最初的骨折不愈合区域显示完全融合,无神经血管缺损,关节活动正常。
讨论:小儿患者骨折不愈合很罕见,因此难以诊断和治疗。由于我们所有的检验都排除了患儿骨折不愈合的主要原因,因此我们采用了通常适用于成年患者的策略来处理该病例,并谨慎对待生长板。
结论:尽管这是单例报告,但它强调了早期诊断的重要性、难以排除某些儿科原因引起的骨流失以及错误诊断/治疗的并发症。它还表明,在接受过多次手术的儿科患者中使用同种异体骨和自体移植生长因子可以产生良好的效果。
