Вителлиформные изменения макулярной области, встречающиеся у взрослых пациентов

Аннотация
Введение. Вителлиформные поражения центральной области сетчатки у взрослых пациентов представляют собой гетерогенную группу заболеваний. В статье описаны различные варианты вителлиформных изменений у взрослых по данным опубликованной литературы.
Материалы и методы. Проанализированы и описаны различные варианты вителлиформных изменений сетчатки у взрослых по данным опубликованной литературы, приведены примеры из собственной клинической практики с использованием мультимодального подхода.
Обсуждение. Вителлиформные изменения центральной области сетчатки могут дебютировать в разном возрасте, протекать моно- и мультифокально, иметь различные стадии деградации вителлиформного материала, маскируясь под другие поражения макулярной зоны и заднего полюса глаза. Многие из этих заболеваний появляются в связи с мутациями в отдельных генах, однако достаточно большая часть случаев расценивается как спорадическая. В настоящее время описаны характерные признаки различных вариантов заболеваний с наличием вителлиформного материала, однако дифференциальная диагностика с другими похожими заболеваниями (отдельными формами возрастной макулярной дегенерации и центральной серозной хориоретинопатией) весьма затруднительна и требует мультимодального офтальмологического подхода, а в ряде случаев — проведения генетических исследований.
Заключение. Вителлиформные поражения центральной области сетчатки, которые встречаются у взрослых пациентов, — это группа заболеваний, которые сложно поддаются диагностике, маскируясь под другие заболевания центрального отдела сетчатки, что требует от врача определённых знаний, возможности проведения мультимодальной диагностики для постановки диагноза и назначения лечения при необходимости.
