Субпороговое микро- и наносекундное лазерное воздействие на сетчатку на ранних стадиях возрастной макулярной дегенерации

Аннотация
На сегодняшний день не существует эффективных методов лечения возрастной макулярной дегенерации на ранних стадиях заболевания. Перспективным может быть способ лечения пациентов с возрастной макулярной дегенерацией с использованием субпорогового лазерного излучения. В данном литературном обзоре приведен анализ результатов применения субпорогового микро- и наносекундного лазерного воздействия на сетчатку при промежуточной стадии возрастной макулярной дегенерации.
Субпороговое диодное микроимпульсное лазерное воздействие на сетчатку при возрастной макулярной дегенерации зарекомендовало себя как безопасное вмешательство, которое позволяет добиться регресса друз макулы. Хотя отдельные авторы при таком лечении отметили улучшение зрительных функций и снижение вероятности развития хориоидальной неоваскуляризации, многоцентровые исследования пока не подтвердили эффективность микроимпульсной терапии как метод профилактики развития неоваскулярной возрастной макулярной дегенерации.
Субпороговое наносекундное лазерное воздействие на сетчатку также безопасно для структур глазного дна, теоретически более избирательно стимулирует естественный биологический и регенеративный потенциал в клетках пигментного и нейроэпителия, за счет ремоделирования экстрацеллюлярного матрикса восстанавливает толщину мембраны Бруха и обмен веществ между наружными слоями сетчатки и хориокапиллярами. Получены обнадеживающие результаты применения наноимпульсной терапии как метода профилактики развития поздних стадий возрастной макулярной дегенерации при отсутствии ретикулярных псевдодруз, однако представляется преждевременной рекомендация данной технологии к широкому клиническому применению — необходимы дальнейшие исследования.
