Middle minilaparotomy авоѵе pubis with laparoscopic aid at myomectomy
Abstract
We elaborated Operation technique for myomectomy by inferior median minilaparotomy with an adequate access of 5 centimeteres and laparoscopic asssistance. We performed 180 operations and experienced that this access is low traumatic but allows surgent's digital contact with utems, traditional Instruments usage and hand sewing on enucleated nodus bed.
Endoscopic transcervical fallopian recannulation for infertility treatment
Abstract
Endoscopically-guided transcervical fallopian recannulation for the treatment of proximal tubal occlusion was performed in 27 patients. 31/38 (81,6±6,3%) tubes were cannulated successfully. Of 25/27(92,6±5,0%) patients at least one tube was successfully treated, the perforation rate was 3,7% (1 patient) and there were 12 pregnancies (48,0%). This procedure has the advantages of visualization of distal tubal segments and uterine cavity. The prognosis is poor when additional distal disease is present.
Ectopic pregnancy - modern aspects of diagnosis and treatment. Clinical and economic effect of the use of modern medical technologies in the treatment of tubal pregnancy
Abstract
The authors present analysis of outcomes 768 operations for ectopic pregnancy by laparoscopy. They are offered the new method blood reinfusion during laparoscopy and the original algorithm of diagnosis an ectopic pregnancy. Laparoscopy is the main method of treatment for ectopic pregnancy.
The experience of laparoscopic-assisted vaginal hysterectomy
Abstract
In our clinic we have been using laparoscopic- assisted vaginal hysterectomy since 1996. Our experience of LAVH amounts to 103 operations. The investigations we carried out confirmed undoubted advantages of laparoscopic-assisted vaginal hysterectomy over other methods in question. The technique used by us resulted in decreasing of complications and extended the number of indications.
Summarizing everything mentioned above we can draw the conclusion that laparoscopic-assisted vaginal hysterectomy has the following advantages: minimal traumatization, favorable cosmetic effect, shot postoperative and rehabilitation period, absence of complications in the late postoperative period, decrease of the risk of intraoperative complications and absence of absolute contraindications. The experience accumulated proves the LAVH technique to be reasonable and efficient.
Analysis of the surgery experience in correction of genitals prolapse realised in the kaliningrad regional centre of endo video surgery
Abstract
The present work contains the analysis of the follow-upgenitals prolapse surgery carried out with the 56 patients in the Kaliningrad regional centre of endo video surgery within the 1998 - 2002. In process of the analysis we have made a comparative assessment of long-term effectiveness of a number of the operations practiced in our centre. We have shown the evolution frequency pattern of such postoperative complications as dysuria and pain syndrome due to the fixed structures tention, as well as prolapse relapse pattern depending on the applied correction technology. We have also shown lack of their dependance on the degree of prolapse evidence. In addition the author has outlined his point of view on the ways of decreasing the amount of relapses in the follow-upgenitals prolapse.
Experience of surgical treatment of manifesting forms of adenomyosis
Abstract
One describes in this article the technique of performing surgeries with saved organs at manifested adenomyosis, one presents the treatment results of women with various clinico-morphological disease forms by means of different surgical methods.
Laparoscopy in differential diagnostics of gynecolodical pelvioperitonitis and acute appendicitis
Abstract
Diagnostic laparoscopy solves all the problems arising from the doubts in diagnosing acute appendicitis and pelvioperitonitis of gynecological genesis. It must be widely used in the cases where clinical and laboratory data do not allow to reliably diagnose acute appendicitis or its absence, this method also allow to specify the nature of the patient's disease.
New opportunities in the treatment of acute infectious and inflammatory diseases of the pelvic organs in women
Abstract
The problem of infectious and inflammatory diseases is one of the most urgent in modern gynecology. With the introduction of laparoscopic techniques, approaches to the diagnosis and treatment of this pathology have changed. We would like to draw your attention to the analysis of our experience in the use of endovideosurgical techniques in the diagnosis and treatment of infectious and inflammatory diseases of the uterus and appendages.
Asessment of laparoscopy role for complex diagnostics and treatment of spreaded forms of purative-inflam- matory diseases of womb appendices
Abstract
Here, data are presented about 150 laparoscopic surgical operations in women with spreaded forms of purativeinflammatory diseases of womb appendices. Laparoscopy is optimal diagnostic and medical intervention in case of gynecological pelvioperitonites. Patients with purative-inflammatory diseases of womb appendixes, inclusions of close organs and wide peritonitis need in the surgical aid trough laparotomic access.
Dufaston (degidrosteron) in treatment of dysfunctional womb bleeding associated with ferrum hyperplasia of endometrium
Abstract
Endometrial hyperplasia, being a fairly common gynecological pathology among women of different age groups, requires timely diagnosis and adequate treatment. Duphaston at a dosage of 10-20 mg from 5 to 25 or from 16 to 25 days of the menstrual cycle showed itself as an effective drug (in 83.3%), with sufficient progestogenic antiproliferative activity, with minimal side effects and a neutral effect on the metabolism of lipids and carbohydrates , which made it possible to use it among patients with various somatic pathologies.
Modern opportunities for diagnostics of chronic endometritis
Abstract
The indices of local immunity were studied by the immunohistochemical method in the endometrium from 151 women with morphological manifestations of chronic endometritis. To confirm the inflammatory process and clarify its nature, the parameters of systemic immunity were studied. According to changes in the parameters of local immunity, three groups of patients were identified. Determination of the parameters of systemic immunity made it possible to confirm chronic endometritis in all women and to identify a group with an autoimmune nature of inflammation.
Possibilities of HRT in solving the problem of psychosocial adaptation of girls with primary estrogen deficiency
Abstract
This study presents the results of testing the hypothesis that girls during the period of normally proceeding puberty experience much more emotional discomfort, difficulties in communication and self-acceptance due to more pronounced hormonal and bodily changes than normally developing boys. Along with this, for the first time, results are presented that indicate some similarity of adaptive responses in boys with normal puberty and in girls with primary estrogen deficiency on the background of hormone replacement therapy (HRT). The development of girls with 45×0-gonadal dysgenesis (with Turner syndrome) on the background of HRT is considered as adaptive to the tasks of age, less than their passport data. Girls with XY-sex reversion with gonadal dysgenesis are assessed as sufficiently adapted due to certain personal and age characteristics.

Methods of reducing blood loss during reconstructive operations on the uterus
Abstract
There can be no doubt about expediency of reconstructive operations on the uterus in young patients. These operations make it possible to preserve reproductive and menstrual function. However, such operations are accompanied by formation of extensive wound surfaces, considerable muscular tissue defects and characterized by high traumatism, quite often impossibility to ensure effective hemostasis and massive blood loss during operation. In this connection a number of complications during early postoperative period is higher than after hysterectomy. Taking into account the above, it is actual to use the surgical techniques which allow to reduce traumatism of operation and ensure effective hemostasis. In present review different methods used for ensuring hemostasis and reducing operative blood loss during reconstructive operations on the uterus are considered.

Influence of surgical intervention on life quality of gynecological patients
Abstract
For investigation of the influence of surgical intervention on life quality of gynecological patients 72 women were inspected, whose clinical picture had as a leading syndrome the chronic pelvis pain (CPP), and 32 women with womb myoma, who did not complain against a pain in the pelvis. Life quality was evaluated before surgical intervention and in 10-12 months after it.
It was detected that due to the big expression of psyching disorders of neurotic level in women with CPPtheir starting level of quality life was considerably lower than in patients with womb myoma.
Inspection results manifested that operative treatment led tc improving life quality evaluating in 10-12 months after surgical intervention in women with womb myoma, but did not lead tc changes in majority of patients with CPP.
Eco in infertility. Inspection of pregnancy and delivery
Abstract
During the period from 1996 to 2002, the IVF department of the Family Planning and Reproduction Center carried out IVF treatment in 2218 infertile patients. Based on many years of experience, the results of using this method are summarized, taking into account the modifications made to it, developed by the staff of the Center. Specific recommendations for the use of endosurgical methods for restoring natural fertility and IVF in cases of female infertility associated with tuboperitoneal factors and myoma are presented. The course of pregnancy and childbirth in patients with IVF programs has been analyzed and approaches have been outlined to reduce their reproductive losses.
The application of simultaneous treatment combined with endovideosurgery in case of gynecological disorders
Abstract
In this article we present considerable date about simultaneous operations in case of combination both surgical and gynecological pathology. The character of operative interactions is also discussed. There we show the results that characterize the application of laparoscopy and hysteroresectoscopy in case of treatment of differently located uterus myoma.

It was attended by about 400 doctors from all over Russia, from Ukraine, Belarus, Kazakhstan, Georgia, Uzbekistan, Latvia, Sweden, Slovenia, Austria and Scotland.
