Retrospective analysis of coronary artery bypass grafting outcomes in the Republic of Mordovia (2022-2024)

Abstract

the article reports on a retrospective analysis of 269 coronary artery bypass grafting (CABG) surgeries performed in the Republic of Mordovia in 2022-2024. The paper examines in detail the impact of surgical tactics (on-pump vs off-pump) on patient mortality. The article reports that during the study period, there was a sharp change in the ratio of methods: the proportion of operations with artificial circulation (IC) decreased from 97,98% to 31,91%, while the frequency of off-pump interventions increased to 68,09%. The article reports that this transition was accompanied by a statistically significant increase in mortality from 0% to 4,26% (p=0,045). The article analyzes in detail that the Pearson correlation coefficient between the use of CPB and mortality was r=-0,98 (p<0,05), demonstrating an almost perfect inverse relationship. The article also states that regression analysis showed that each 10% decrease in the proportion of operations with CPB led to an increase in mortality by 0,625%. The article emphasizes that demographic factors (mean age 64±5,2 years, 87% men) did not have a significant impact on the outcomes. The article concludes with a discussion of the need for an individualized approach to the choice of surgical tactics, especially in patients with complex coronary artery disease.
