激光手术治疗有不良产科-妇科病史患者子宫腔粘连的疗效评估:STROBE分析

论证。在女性不孕症的构成中,子宫因素的发生率可达15%;由于宫腔操作日益增多,子宫腔粘连及Asherman综合征的患病率持续上升。尽管治疗效果总体良好,但Asherman综合征在接受不同类型手术治疗后,其复发风险仍然较高,可达30%–66%。当前医学面临的重要任务之一是开发更有效的方法,以抑制纤维化、促进再生并恢复功能性子宫内膜。
目的。评估对具有不良产科-妇科病史的子宫腔粘连或Asherman综合征患者采用综合性激光手术治疗方案的疗效。
材料与方法。本研究在Sechenov University的Clinic of Obstetrics and Gynecology n.a. V.F. Snegirev开展,为一项回顾性队列研究,回顾分析2014–2021年因子宫腔粘连或Asherman综合征导致子宫性不孕的327例患者病历资料。诊断依据为盆腔器官超声检查与门诊宫腔镜检查。纳入患者均具有不良产科-妇科病史,包括妊娠失败、多次宫腔操作、分娩及产褥期并发症。所有患者接受激光能量对子宫腔粘连的消融处理,并于术后宫腔内注入抗粘连凝胶。术后第3个月和第6个月进行随访门诊宫腔镜检查以评估治疗结果。
结果。294例患者(90.0%)获得远期随访数据。在术后第3个月和第6个月的随访中,有20例患者(6.7%)被发现存在少量子宫腔粘连,其中18例再次接受宫腔镜下激光粘连分离术并宫腔内注入抗粘连凝胶,另有2例需进行三阶段宫腔镜激光手术。在接受宫腔内抗粘连凝胶注射的患者中,未观察到子宫腔粘连复发。178例存在月经紊乱的患者中,136例(76.0%)恢复正常月经,42例(24.0%)月经规律但量少。在计划于未来一年内妊娠的300名女性中,研究者获得了239名的随访资料,其中155名(65.0%)已妊娠,132名已分娩,46名继续接受辅助生殖治疗。
结论。激光能量消融联合宫腔注入抗粘连凝胶在治疗子宫腔粘连中被证实具有较高的疗效 (降低复发率、恢复月经功能及提高妊娠可能性)。结果强调,为改善患者的生殖健康,应采取综合性的方法对宫腔粘连进行及时诊断、治疗和预防。
