Abstract
Российский фармакологический рынок весьма насыщен. Практически каждая лекарственная группа включает несколько близких по свойствам препаратов, реальные различия между которыми относительно невелики и часто понятны лишь узким специалистам. Огромное количество оригинальных лекарственных препаратов дополняется на порядок большим числом генериков. Практикующему врачу непросто сделать выбор среди этого многообразия, учитывая агрессивную рекламу, а также обилие разнородной и порой необъективной информации, предоставляемой фармакологическими фирмами и сотрудничающими с ними экспертами. Важнейшее оружие, которое может помочь в данной ситуации, – использование единых и ясных критериев оценки эффективности и безопасности препарата, основанных на проведении контролируемых исследований и длительной оценки опыта использования конкретного лекарственного средства в реальной клинической практике. С этой позиции мы предлагаем рассмотреть ситуацию с обезболивающими препаратами, которые применяются в российской терапевтической практике. Интерес к анальгетикам не случаен, ведь эти лекарственные средства наиболее широко используются в клинической практике и повседневной жизни. Причина их популярности очевидна – купирование боли, являющейся наиболее тягостным проявлением болезни, относится к числу первоочередных задач для врача любой терапевтической специальности. Наилучшей моделью, которая может быть использована для изучения клинических достоинств лекарственных препаратов, обладающих обезболивающим действием, могут считаться заболевания суставов и позвоночника. Такая патология, как остеоартроз и дорсопатии, является наиболее частой (после заболеваний сердечно-сосудистой системы) причиной обращения за медицинской помощью и главным основанием для длительного использования анальгетиков. С другой стороны, больные, страдающие хроническими ревматическими заболеваниями, – в своем большинстве лица пожилого возраста, имеющие множественную коморбидную патологию, а значит, и факторы риска лекарственных осложнений, которые могут возникать на фоне приема обезболивающих средств.
