Abstract
Трансформация ботулинического токсина (БТ) из яда в лекарство произошла усилиями доктора Alan Scott [7]. В поисках нехирургического метода лечения косоглазия он получил от Schantz очищенный кристаллический БТ типа А и использовал его в экспериментах на обезьянах, а через 7 лет у людей. В своей исторической работе он продемонстрировал безопасность и эффективность внутримышечного введения БТ типа А для лечения больных косоглазием. И в 1989 г. БТ типа А в виде препарата "Ботокс" (Allergan, Inc.) одобрен FDA для клинического применения у больных с заболеванием мышц глаз.Сегодня прицельное введение терапевтических доз БТ используют для лечения многих нейромышечных расстройств. БТ нашел свое применение в таких специальностях, как офтальмология, неврология, проктология, гастроэнтерология и косметология.В урологии БТ в основном применяют для лечения больных с симптомами нижних мочевых путей вследствие нейрогенной и идиопатической детрузорной гиперактивности, детрузорно-сфинктерной диссинергии (ДСД) и других спастических состояний сфинктера уретры и со снижением или отсутствием сокращения детрузора. Дополнительными показаниями для использования БТ у урологических больных являются хронические болевые синдромы и доброкачественная гиперплазия простаты.БТ является альтернативным методом лечения различных нарушений функции нижних мочевых путей. Ограниченный, но растущий клинический опыт показывает, что временная хемоденервация с полным или частичным подавлением нейрональной активности в органе-мишени может быть достигнута с минимальным риском. Несмотря на сохранение большого количества вопросов в отношении оптимального применения БТ в урологии, эта возможность откроет урологам новые перспективы для лечения сложных нарушений мочеиспускания.
