Abstract
Признание лимфомы Ходжкина (ЛХ) потенциально излечимым заболеванием в 1970-е годы во многом произошло благодаря введению в широкую лечебную практику полихимиотерапии при распространенных стадиях заболевания, в первую очередь программы МОРР. В 1940–50-х годах был синтезирован и начал применяться для лечения онкологических больных ряд химиопрепаратов с разным механизмом действия: эмбихин (мустарген), хлорамбуцил, циклофосфамид, CCNU, винкристин, винбластин, метотрексат, в том числе и прокарбазин (натулан), а также синтетический стероидный гормон преднизолон. Уже первые попытки применения этих химиопрепаратов в монорежиме у больных с ЛХ показали их эффективность у больных с распространенными стадиями заболевания. Однако несмотря на довольно высокую частоту непосредственных ответов, ремиссии при монохимиотерапии, в том числе и при лечении прокарбазином, были кратковременными и не улучшили общей выживаемости больных. Удачной оказалась лишь предложенная в 1964 г. комбинация из 4 химиопрепаратов – мустаргена, винкристина, прокарбазина и преднизолона, повторяющаяся с постоянными 2-недельными интервалами – схема МОРР. Ключевыми моментами для создания этой схемы явились замена метотрексата в схеме МОМР (мустарген, винкристин, метотрексат, преднизолон) на прокарбазин и продолжение лечения до 6 циклов
