Musculoskeletal Diseases as Social and Economic Problem
Abstract
Dynamics of musculoskeletal diseases incidence among adult population for the period 2006— 2010 is presented . Social and economic significance of orthopedic pathology is shown. Main directions for specialized care quality improvement are proposed.

The analysis of rendering of specialized medical care by the patient with damages and hand diseases to the city of Moscow and ways of its optimization
Abstract
In article the analysis of a condition of rendering of the specialized help by the patient with damages and hand diseases is carried out. In particular the structure and volume of rendered specialized medical care by such patient in the conditions of offices of surgery of a hand of City clinical hospitals № 4 and 6 of Department of health care of the city of Moscow during the period from 2006 to 2011 are studied. The reasoned arguments about expediency of creation of the Moscow City Center of Hand Surgery.

Puncture Transpedicular Fixation in Surgical Treatment of Thoracic and Lumbar Vertebrae Injuries
Abstract
Results of transpedicular fixation (TPF) performed to 113 patients with injuries of thoracic and lumbar vertebrae are presented. In 26 patients original puncture TPF was performed, in 14 cases the reposition system was used. Analysis of treatment results showed high efficacy of puncture TPF. Duration of surgical intervention and volume of blood loss decreased reliably, cosmetic defect was less marked.

Modern Synthetic Substitute of Bone Tissue
Abstract
Potentialities of synthetic bone substitutes application in vertebrology, especially in cervical spine surgery is demonstrated. From 2010 to 2011 thirty seven patients (21 — 70 years) with spine injuries and degenerative diseases were operated on. During surgical intervention subtotal resection of cervical vertebra body was performed. BoneMedik-S block of appropriate size was inserted into the defect. In all cases the final step of operation was stable fixation of segment by metal plate and screws. It was shown that modern synthetic biomaterials for substitution of bone tissue defects enabled to shorten the volume of surgical intervention and contributed to the formation of natural spongy human bone structure at minimum terms.

Complex Diagnosis of Myofascial Lumbosacral Pain Syndrome in Athletes and Ballet Dancers
Abstract
Myofascial lumbosacral pain syndrome is a common disorder in athletes and ballet dancers. Diagnosis of this disorder is rather difficult as usually several muscles are involved and also the combination with degenerative diseases is possible. Complex clinical and instrumental examination using ultrasonography, computed tomography and surface electromyography enables to make an accurate diagnosis and specify the severity of damage.

Standardized Assessment of Femoral Head and Neck Revascularization Outcomes in Degenerative Dystrophic Hip Joint Diseases at Late Terms
Abstract
Analysis of femoral head and neck revascularization efficacy at late terms (7 to 20 years) was performed using "Standardized assessment of outcomes in degenerative dystrophic diseases of loco-motor system" (SAO-3). Treatment results were assessed for 3 groups of patients with degenerative dystrophic hip joints diseases (DDHJD). Grouping ofpatients was based on the type and stage ofpathology, age of patients, terms of examination (preoperatively and in 1, 3, 5, 10, 15, 20 years after surgical intervention). Group I included 9 patients with early pre-radiologic stage of femoral head avascular necrosis (FHAN); group II — 5 patients operated on at other stages of FHAN, group III — 6 patients with coxarthrosis and cystic remodeling ofjoining bones. In patients from group I positive stable outcome was present in 20 years after operation that allowed to state their recovery. In patients with other stages of DDHJD positive surgical outcome was preserved for up to 15 years.

Chronic Venous Insufficiency and Gonarthrosis
Abstract
The rate of previously not diagnosed chronic insufficiency in patients with knee osteoarthrosis was determined. The effect of treatment measures directed to elimination of venous insufficiency was evaluated. It was shown that in 48.8% of patients chronic venous insufficiency was not diagnosed by orthopaedic surgeons. Observance of therapeutic regimen, use of elastic compression and phlebotropic drugs contributed to the elimination of gonarthrosis symptoms and improved the quality of life (by KOOS scale).

Arthromedullar Bypass at Knee Crucial Ligaments Endoprosthesis
Abstract
Arthromedullar ligamentous implant that provides the insertion of intraosseous content with pronounced lubrication and antioxidant activity into joint cavity has been proposed. Use of that implant during surgery for knee ligaments rupture (18 patients aged 18 — 52 years) side by side with mechanical fixation enabled to decrease considerably friction between the articular surfaces, contributed to a more rapid full value restoration of joint function and prevented the posttraumatic osteoarthrosis development.

Experience in 500 Total Knee Replacements
Abstract
Experience of CITO joint replacement department in knee arthroplasty with implants from various manufacturers was analyzed. Implants of three modifications were used: basic, with posterior stabilizer and "semiconstrained" one. Tactics for a choice of optimum construction depending on a disease, gender, degree and type of contracture, degree and type of limb axis curvature was demonstrated and substantiated on the example of 500 operations performed to 426 patients with various knee joint pathology during the period from 2000 to 2010. By the data of clinical and roentgenologic analysis excellent result was achieved in 90 (21%), good inv 277 (65%), satisfactory in 51 (12%) and poor in 8 (2%) patients. This treatment tactics may be considered as an adequate one.

Osteochondritis Dissecans of Femoral Condyles in Children and Adolescents
Abstract
Experience in treatment of 76 children with K^ig's desease during the period from 1992 to 2011 is presented. Examination plan included roentgenography, CT, US, MRT and arthroscopy. In 41 children 1 st and 2 nd stages of the disease was diagnosed, in 9 and 26 patients 3 rd and 4 th stages, respectively. Defect size averaged 3.17 sq. cm with 6.8 mm depth. In 1 st and 2 nd stages of the disease osteoperforation was performed, in 3 rd and 4 th stages — intrafocal osteoperforation of the defect bottom and cartilaginous defect plasty with Chondro Gide collagen membrane was applied. All interventions were performed arthroscopically. Excellent results were achieved in 69 (90.7%) patients, good in 5 (6.5%) and satisfactory in 2 (2.6%) patients.

Tribochemical Component of Oxidative Stress Development at Artificial Joints Implantation. Part 3. Inhibition of Radical-Forming and Antiproliferative Ability of Wear Particles by Antioxidants and Bone Fat
Abstract
Tsepalov Inhibition ability of certain antioxidants and bone fat upon the oxidative properties of wear particles was studied. It was shown that at presence of wear particles use of BHT and б -tocoferol resulted in an inhibition of cumene oxidation. Treatment of orthopaedic alloys particles with bone fat was also accompanied by complete inhibition of oxidation reaction and duration of effect depended on the content of bone fat antioxidants. Addition of bone fat into mixture of alloy and polyethylene particles prevented the development of free-radical reactions ana potential oxidative destruction ofpolymer. Treatment of alloy particles with bone fat considerably reduced their negative influence upon human bone marrow osteogenic cells.

Use of Enoxaparin and Dabigatran for Thrombosis Prevention After Total Knee Replacement
Abstract
Comparative analysis of dabigatran etaxilate and enoxaparin sodium use for prevention of vena cava inferior system thrombosis in patients after total knee replacement in early postoperative period was performed. In 74 patients (1 st group) dabigatran etaxilate and in 127 (2 nd group) enoxaparin sodium was used as a preventive measure. The rate of thrombosis made up 1.35% and 2.4% in the first and second groups, respectively. No significant difference in the volume of perioperative blood loss between the groups was noted. No other hemorrhagic complications were observed. It was shown that in more convenient peroral administration the dabigatran etaxilate efficacy and safety was comparable to enoxaparin sodium.

Use of Multilayer Thin-Film Coatings for Complete Restoration of Integument in Its Deep Damage (Experimental Study)
Abstract
The effect of multilayer thin-film wound coatings upon regeneration of skin in its deep damage was studied 18 Chinchilla rabbits. Full-layer skin wounds 40 x 40 mm were inflicted on the back of the animals. In experimental group of animals the wounds were closed by multilayer coatings consisting of 50 aluminum metalized lavsan films. In control group one layer coating with thin aluminum plate was used. In all control animals the pyonecrotic wound processes developed within 1 — 2 weeks after intervention. In experimental rabbits the wound healing process showed no clinical signs of inflammation. In 6 weeks a full value regenerate in the zone of deep skin damage was formed. Its thickness and structure was typical of normal skin. Principle of multilayer coatings is perspective for the elaboration of coatings at treatment of deep and small wounds of various etiology when high quality of dermal regenerate is required.
