THE ROLE OF HEALTH PROFESSIONALS IN MEETING THE CHALLENGES OF HEALTHCARE — RHEUMATIC AND MUSCULOSKELETAL DISEASES IN THE 21ST CENTURY
Abstract
Developed countries throughout the western world face significant challenges in meeting rising demands for healthcare. There is a growing need to build services that offer a cost effective and consistent quality of care delivered by a highly skilled workforce. Clinicians working in the field of Rheumatic and Musculoskeletal Diseases will face increasing demands that potentially threaten their ability to deliver high quality care, prompt, yet cost effective care designed for the 21 st Century. Countries that have a well-developed healthcare system within the Western world have already begun the process of addressing these challenges, using a mix of approaches such as service re-design, changes in the workforce provision and a greater focus on delivering a patient focussed approach to care. Service re-design has often used the current workforce in new ways or advanced the skills and competencies of healthcare professionals (including nurses, physiotherapists, occupational therapists, podiatrists and psychologists). Whilst advancing the roles of Health Professional (HP) this process does replace the expertise of their medical colleagues but complements, enabling the physicians to carry out important diagnostic and management decision on complex patients. This paper explores the drivers for change and considers the issues related to changing service delivery to optimise care by applying a strong multi-disciplinary approach to the future of healthcare for Rheumatic and Musculoskeletal Diseases (RMD).

FEATURES OF A CLINICAL CURRENT OF A CHRONIC GENERALIZED PERIODONTAL DISEASE AT MEN WITH PARTIAL ANDROGEN DEFICIENCY IN AGING MALE (PADAM SYNDROME)
Abstract
It is known that the level of sex hormones plays an important role in the pathogenesis of chronic generalized periodontitis in female; in men this problem is less investigated. The aim of this study is to reveal the features of the clinical course of CGP in men with partial androgen deficiency of aging male (PADAM). The studies found that the chronic generalized periodontitis in men with laboratory-established deficiency of sex hormones is characterized by severe clinical course and poor prognosis.

DEFINITION OF MEDICAL TACTICS OF GONARTHROSIS TREATMENT
Abstract
In the article is given the algorithm of the selection of the tactics of the treatment of knee osteoarthritis on the basis of the evaluation of X-ray changes, knee dysfunctions and data of arthroskopy. The importance of the personified approach to the selection of tactics is emphasized. Based on the example of 517 observations is shown the role of arthroskopy in the determination of indications to knee arthroplasty, and also the possibility of achieving good results with the failure of knee arthroplasty in patients with the relatively safe articulate cartilage.

EFFICACY AND TOLERABILITY OF THE “NISE” IN THE COMPLEX CONSERVATIVE TREATMENT OF GONARTHROSIS
Abstract
Analysis of the results of the complex conservative treatment, carried out in 182 patients with knee osteoarthritis, shown effectiveness and a good tolerability of the “Nise”. Complemented treatment of the patients was depended on specifity of the clinical picture of osteoarthritis. There were no cases of hepatotoxity.

INTRAARTICULAR DRUG INJECTIONS IN THE TREATMENT OF JOINT DISEASES
Abstract
The results of applying the intra-articular injection therapy within the five-year period (with 2008 until 2012) are analyzed in 11722 patients with the diseases of joints. The results of the clinical effectiveness of a number of preparations are given. The complications and the undesirable phenomena, which arose during their application are discussed.

SIGNIFICANCE OF AUTONOMIC DYSFUNCTION IN THE PATHOGENESIS OF VARIOUS FORMS OF THE SYNDROME DIABETIC FOOT
Abstract
In this article a study of the connection of the types of the vegetative dysfunction of patients and various forms of the syndrome of diabetic foot is conducted. Is shown close correlation between such criteria as the surface tension of blood plasma and the index of vegetative equilibrium, with the characteristic bone-articular lesions of lower extremities, in connection with which these indices may be used for setting of diagnosis and determining the content of therapeutic- rehabilitative measures.

The aim of the study was to estimate the complex treatment efficiency in patients with TMJ internal disorder. Releasing an articular disc from the fibrous unions, applying special mouthpieces, employing a hydraulic pressure method and implementing mechanical treatment assure the possibility for the restoration of anatomic and functional interrelations of joint units, the normalization or the significant increase in motion of joint in lower jaw.

BONE MINERAL DENSITY ESTIMATION OF WOMEN IN THE OLDER AGE GROUPS
Abstract
The bone mineral density was estimated in women (n=1463) of the older age groups. DXA examination included the lumbar spine, proximal femur and distal forearm. The results of study shows the usefulness of the forearm bone DXA in women older than 70 years, especially in the presence of marked degenerative changes in bones and joints.

The experience of the application of orthesis in 602 patients with knee osteoarthritis of different degree of gravity on the base of the Arthrology hospital. It was shown the advantage of the including of orthesotherapy in complex of osteoarthritis treatment, which made possible to reduce the dose of NSAIDs, to decrease the pain and to improve the quality of life.

MICROCIRCULATION OF THE DENTAL PULP IN THE ELDERLY
Abstract
A study of microcirculation of the pulp of the tooth in the elderly without apparent somatic diseases than with young people using the LDF. The analysis of LDF-gram shows stagnation in the pulp of a tooth in the elderly. When evaluating the mechanisms of regulation of blood flow in the dental pulp myogenic fluctuations rhythms in elderly patients indicate metabolic acidosis, decreased blood flow and pressure in the microvasculature of the dental pulp.

THE USE OF THE PANORAMIC RADIOGRAPHS OF THE JAWS FOR THE EARLY DIAGNOSIS OF OSTEOPOROSIS IN DENTAL PRACTICE
Abstract
Article is devoted to the early diagnostics of osteoporosis during the routine dental care. The quality of mandibular bone mass was assessed and quantified with the use of panoramic radiographs. The comparative analysis of panoramic radiographs of patients with low BMD with the computer tomography data was held. It was shown that the use of the mandibular cortex index (MCI) may be used as a diagnostic criteria of postmenopausal osteoporosis.

TOTAL HIP ARTHROPLASTY IN ADVANCED AGE
Abstract
Total hip arthroplasty in elderly patients was performed in 520 patients. Basically, hip replacement was performed on Ce-mentless technology. Long-term results of treatment were studied in a period of 5 years. Analysis of long-term outcome was studied using X-ray and scale Harris survey. Positive results for cemented prostheses were prepared in 89.5% of cases.

SMALL-DATA STATISTICS ANALYSIS IN MEDICAL STUDIES
Abstract
The experimentally was found characteristics of statistical criteria. As a result, calculated the value of the statistics by W. Ansari-Bradly and K. Klotz. For each source of statistics calculated normal approximation (Z-statistics) and the significance level of p of the null hypothesis of no difference in the spread of the values of the two samples. At p>0.05 the null hypothesis can be accepted. Suggested methods of mathematical statistics can be confirming the accuracy of the differences of the results, even in small groups of observations, if the differences are significant enough. We used medical cases of patients with joint and bone pathology.
