Abstract

An outpatient surgery center was set up at Krasnodar City Clinical Hospital Three in 1998, which introduced into practice all types of endoscopic operations for cholelithiasis, such as papillotomy, transpapillary interventions, and laparoscopic cholecystectomy (LCE). An algorithm and principles were developed for the treatment of cholelithiasis at a short-stay hospital (SSH). In 1998-2004, a total of 1361 patients were operated on and 1465 operations: 1305 laparoscopic cholecystectomies, 251 endoscopic retrograde cholangiopancreatographies, and 160 endoscopic papillotomies were performed.
Failures of LCE were its duration of more than 2 hours (18%), conversion (0.3%), and complications (0.5%), among which there were damages to the hepaticocholedochus and two hollow organs. Transpapillary operations resulted in failures (2%) and complications (6%).
Thus, there were complications (3%), conversions (0.6%), and no deaths after endosurgical treatment for cholelithiasis at the SSH. The mean bed/days were in the range of 1.7-4.5.
