儿童脊柱病理性骨折 (文献综述及单中心队列的临床和形态学分析)

论证:儿童病理性椎体骨折在炎症、肿瘤和营养不良病变中较为少见。
目的是分析儿童病理性脊柱骨折的临床表现和形态结构特点。
材料与方法。对62例2至17岁儿童在诊所进行了检查和手术病理性椎体骨折的临床及放射学特征、
形态学结构进行了研究。
结果。入院时儿童平均年龄为10岁,无发现性别特征。胸椎占位性病变(78%),多见于生理后凸位Th7-8
的顶点。在10例中,发现多个病变,包括骨骼的其他部分的病理。非机械性(非运动)背痛(69%)、
触诊疼痛(34%)和局部脊柱畸形(27%),临床症状以平均24°的局部脊椎后凸为主。11例患者(18%)
出现神经功能缺损,其中9例骨折是肿瘤过程所致。在16%的病例中,椎体骨折是意外的放射发现。
在病理放射症状中,所有病例中椎体高度均有下降,其中12例为椎体塌陷。除了冲击变形之外,
还有各种各样的毁坏方式。对56例患者进行了诊断和治疗干预;在6名儿童中,操作仅限于环钻活检。
50%的病例中,病理性骨折是由炎症过程引起的,42%是由肿瘤引起的,31%是恶性的。
结论。儿童脊柱的病理性骨折应被视为一种综合征,在大多数情况下是基于炎症或肿瘤过程。由于肿
瘤的高频率,包括恶性,过程,积极的侵入性诊断是必要的。治疗策略取决于临床、放射和病理形态
学特征。
