Роль ретроэлементов генома человека в развитии сахарного диабета 1-го типа

Ретроэлементы (ретротранспозоны и эндогенные ретровирусы) представляют собой класс мобильных генетических элементов, которые перемещаются в геноме путём вставок собственных обратнотранскрибированных транскриптов. Они служат драйверами эпигенетической регуляции, поэтому индивидуальные особенности распределения ретроэлементов в геноме влияют на развитие многофакторных заболеваний. Сахарный диабет 1-го типа — многофакторное заболевание с иммунным ответом против β-клеток поджелудочной железы. Роль наследственности в развитии болезни оценивается в 88%, при этом определяется роль аллельных вариантов различных генов. Выделяют также другие специфические типы сахарного диабета, которые составляют более 2% случаев сахарного диабета и бывают моногенными болезнями с аутосомно-доминантным типом наследования вследствие герминальных мутаций в генах MODY, включающих HNF4A, GCK, HNF1A, HNF1B. У большинства больных сахарным диабетом 1-го типа обнаруживают белковый продукт и рибонуклеиновую кислоту (РНК) ингибитора инсулина HERV-W-Env, что обусловлено аномальной экспрессией эндогенного ретровируса человека (HERV — от англ. human endogenous retrovirus). Сделано предположение о роли ретроэлементов в развитии сахарного диабета 1-го типа. Это обусловлено их вовлечением в филогенетическое формирование эндокринной системы, поскольку в эволюции ретроэлементы оказались источниками регуляторных последовательностей генов гормонов, ядерных рецепторов гормонов и сайтов связывания с ними. Сахарный диабет 1-го типа ассоциирован со встраиванием HERV в область генов HLA-DQ, с аллельными вариантами и размерами вариабельных тандемных повторов VNTR (входящих в состав ретроэлементов SVA), которые регулируют экспрессию гена инсулина и других гормонов. По этой причине вероятно, что в основе развития сахарного диабета 1-го типа могут лежать индивидуальные особенности распределения HERV в геноме человека и их динамические изменения в онтогенезе. HERV играют роль в этиопатогенезе сахарного диабета также посредством активации аутоиммунного ответа, пусковыми факторами которого становятся экзогенные вирусные инфекции и стрессовые воздействия. Таким образом, ретроэлементы участвуют в различных механизмах развития сахарного диабета 1-го типа, что отражает их глобальное регуляторное влияние на эндокринную регуляцию.
