终末期慢性肾病患者合并缺血性卒中的发病机制特点及管理策略

终末期慢性肾病(CKD)患者的急性脑血液循环障碍是一个重要但研究不足的问题。主要挑战在于缺乏经过验证的临床指南和大规模多中心研究,以确定这些疾病合并存在时的有效治疗方法。这类患者很少被纳入临床研究,并具有较高的并发症风险。在接受血液透析的终末期慢性肾病患者中,高达三分之一的卒中发生在住院期间,被归类为院内卒中。评估慢性肾病患者在使用造影剂进行检查时的相关风险是一个重要问题。肾功能损害显著增加了急性缺血性卒中早期阶段的不良结局发生率,但系统性溶栓治疗并未加重结局,也未增加颅内出血的风险。
本文综述了2015年至2022年期间有关终末期慢性肾病患者发生缺血性卒中的回顾性和前瞻性研究以及系统性综述。这些研究收录于MEDLINE、PubMed、Google Scholar、Scopus和eLIBRARY等文献数据库中。仅选取发表于同行评审学术期刊的文章用于分析。检索策略基于关键词组合:“缺血性卒中”、 “慢性肾病”、“血液透析”、“溶栓治疗”、“血管造影”。手动审查了所有相关文献和系统性综述中的参考文献清单。共筛选947篇文献标题,96篇全文文章,其中38篇被纳入本综述。
本文分析了终末期慢性肾病患者(接受肾替代治疗)缺血性卒中的病理生理机制特点和风险因素, 以及该人群接受放射性对比研究和系统性溶栓治疗的可行性和特征。
