Перспективы фармакологической валидации использования тромбоцитов в качестве «периферической модели» нейронов

Депрессивные расстройства часто встречаются у пациентов с патологией сердечно-сосудистой системы и являются предиктором развития тромботических событий, таких как инфаркт миокарда, острое нарушение мозгового кровотока по ишемическому типу, тромбоэмболия легочной артерии. Имеются основания полагать, что это связано со структурным и биохимическим родством тромбоцитов и нейронов головного мозга, что позволяет рассматривать тромбоциты как маркер патологии центральной нервной системы. Цель настоящего обзора — оценка взаимосвязи системы гемостаза и развития депрессивных расстройств с позиций использования тромбоцитов в качестве «периферической модели» нейронов и оценки эффективности лекарственной терапии депрессии. Работа проведена в соответствии с рекомендациями Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA). Для данного обзора систематический поиск литературы проводился с использованием баз данных PubMed, Cochrane и CINAHL за период с 2018 по 2023 г., по ключевым словам: «гемостаз», «острое нарушение мозгового кровообращения», «депрессия», «депрессивные расстройства», «тромбоциты», «сердечно-сосудистые заболевания». Полученные данные свидетельствуют о клинической связи между депрессивными расстройствами и сосудистыми событиями, а также о структурной схожести тромбоцитов и клеток ЦНС за счет однородности следующих белков: транспортеры и рецепторы серотонина или 5-гидрокситриптамина, белок-предшественник амилоида и нейротрофический фактор мозга, которые ранее считались специфическими нейронными белками. Кроме этого, существует взаимосвязь между динамикой гемостаза и лекарственной терапией депрессий. Критически проанализированы изменения гемостаза с точки зрения активации тромбоцитов у пациентов с депрессией при сосудистых заболеваниях и лекарственной терапией депрессий. Механизмы тромбоцитарной индукции, представленные в литературе, разнообразны и требуют дальнейшего изучения. Рациональное изучение путей активации тромбоцитов у пациентов с депрессивными расстройствами позволит получить новые представления о сути молекулярных механизмов, лежащих в основе связи гемостаза у пациентов с депрессией при различных сосудистых патологиях до и после лекарственной терапии. Активация тромбоцитов у пациентов, имеющих депрессию, и динамика изменений показателей системы гемостаза на фоне лечения депрессивных расстройств позволяет рассматривать гемостаз в качестве периферического маркера работы центральной нервной системы и курсовой фармакотерапии.
