Dynamics of complex indicators of assessment of palliative care provided: a study of 2021-2022

Abstract

The purpose of this work is to assess the subjective perception of satisfaction of palliative care users. Materials and methods. The material for the analysis was the results of a sociological survey of palliative care users over the age of 18, conducted in September-October 2022 by personal interviews at the place of residence of representatives of these target groups. A pilot study was previously conducted: 10 diagnostic expert interviews with representatives of the professional community and public organizations related to palliative care in the Russian Federation: and a pilot study of the developed questionnaires with the participation of 100 respondents. The study also carried out the calculation of complex indicators to calculate their subsequent dynamics. Results.  According to the results of the survey, complex indicators of the importance of the main parameters of the PMP were calculated for 9 main blocks. The study showed that the comprehensive assessment of the work of medical personnel in the provision of PMP increased by 7,1% – from 7,4 points in 2021 to 7,9 points on a 9-point scale in 2022. The increase in the regularity of rounds by the attending/on-duty doctor during hospital stay and the fact that the same medical worker conducted the observation was particularly strongly influenced by the growth of the comprehensive assessment (the increase in the parameter estimate was 11,3%). The study also showed that from the point of view of patients, the speed of providing PMP has significantly increased, the complex indicator for this block of issues has increased by 14,8% over the past year. The study also revealed an increase in patient satisfaction with awareness of the treatment process, it increased from 5,8 points in 2021 to 6,2 points in 2022, with the maximum possible number of points – 7. The patients especially appreciated the openness of medical workers in obtaining information about their condition and information about the qualifications of medical personnel. Of course, the complex indicator of satisfaction with the parameters of personal participation of patients in the organization of PMP gave a positive trend in all components. There is a positive dynamic in the assessment of patients with a comprehensive indicator of satisfaction with social and spiritual aspects of treatment, its growth was mainly provided by such components as professional psychological assistance and the opportunity to communicate with representatives of the clergy. Conclusion. The results of the survey of palliative care recipients demonstrate a high level of satisfaction of respondents with all parameters of palliative care. Comprehensive assessment parameters allow us to assess the dynamics of the main areas of assessment of the quality of PMP from the point of view of patients and cover all the main areas of palliative care, they allow us to track changes in their values over time, as well as to calculate them in the context of various groups of citizens, target audiences, and most importantly, to determine the development potential of a particular parameter of the quality of PMP.

The problem of anesthesia of regional society (on the example of the Republic of Mordovia)

Abstract

Target: the aim is to identify the factors contributing to the involvement of the population of the Republic of Mordovia in drug use.Materials and methods. The study was conducted among residents of the Republic of Mordovia aged 14 to 60 years. Here we used a multi-stage sampling with quota selection of respondents at the last stage. A two-stage cluster analysis was applied to determine the factors of population involvement in drug use.Results. The study showed that the population is aware of the seriousness of the problem of drug addiction in the country. However, other questions require priority resolution at the respondent’s place of residence.The article compares the indicators of the spread of non-medical drug use according to official medical statistics and the results of sociological research. There is a discrepancy between the values of the prevalence of non-medical drug use according to the data of the Ministry of Health of the Republic of Mordovia and the data of sociological studies. According to the study, the proportion of the population of Mordovia who consume drugs is 0,7%. The main factors of involvement in drug use were the social environment, the influence of the Internet, and the availability of narcotic drugs.Conclusion. According to the analysis, two clusters are identified. The first one forms the majority (90%) of the inhabitants of Mordovia. It includes those who have not received offers to try drugs, do not use the Internet to get information about drugs and who do not have drug users in their social environment. The second cluster (10%) included respondents who received an offer to try drugs, who were familiar with drug users and resorted to the Internet for drug information. The socio-demographic characteristics of the second cluster differ from the general sample, men prevail here, and the population in the age ranges from 21 to 25 years, 26–30 years and 31–35 years.

Analysis of drug provision for patients with broncho-obstructive diseases in the Russian Federation

Abstract

Introduction. The article presents data from an analysis of drug provision for patients with bronchial asthma and chronic obstructive pulmonary disease, both in inpatient conditions and at the stage of outpatient treatment. The object of the study was the provision of medications to patients with broncho-obstructive diseases in the constituent entities of the Russian Federation. The purpose  of our study was to study drug provision for patients with broncho-obstructive diseases. Why the following research methods were used: purchases of medicines in federal districts and some constituent entities of the Russian Federation in 2019–2022 were analyzed, the amount of funds per patient diagnosed with bronchial asthma or chronic obstructive pulmonary disease was calculated. Separately, we analyzed the purchases of short- and long-acting bronchodilators, as well as mono- and combination drugs and monoclonal antibody drugs for the treatment of patients with bronchial asthma. Data on drug purchases were analyzed using the website zakupki.gov.ru; information on the number of patients in federal districts and constituent entities of the Russian Federation was taken from statistical materials of the Russian Ministry of Health. Results. We found a multiple difference between federal districts in the amount of funds per patient in both 2019 and 2022, and differentiation between federal districts also turned out to be the case when analyzing preferential drug coverage. The dynamics of the amounts of funds spent by entities on the purchase of drugs for the treatment of patients in outpatient settings turned out to be multidirectional: some entities not only did not increase these amounts in 2012 compared to 2019, but, on the contrary, decreased them. At the same time, the difference between the subjects of the same federal district reaches tenfold values. Conclusion. Patients with broncho-obstructive diseases living in different federal districts of the country have different drug coverage both in outpatient and inpatient treatment settings. To improve the drug supply for these patients, it is necessary to introduce a unified methodology for calculating the planned need for drugs and oblige the subjects to provide patients with chronic obstructive pulmonary disease with drugs at the expense of the budgets of the subjects.

Medical support for major international sports events: a literature rewiew

Abstract

Introduction. The article carries out a critical review of the literature and updates the emphasis in organizing the preparation and holding of major sporting events from the point of view of a healthcare organizer.Target: to identify areas of activity that require further improvement and development in the organization of medical support for major sporting events.Materials and methods. An analysis of the available literary sources of databases was carried out: RSCI, RusMed, Pubmed, Web of Science.Results. The most pressing groups of issues for further study and solution have been identified: the uniqueness, significance and complexity of sporting events; statistics and structure of diseases and injuries; features of the structure and course of medical conditions at sports facilities and events; organizing interaction between different services within the healthcare system and interdepartmental interaction; prevention and monitoring of infectious diseases; features of training of medical personnel.Conclusion. The holding of major sporting events, in turn, is a driver for the development of the healthcare system in the host city and country. Preparing for such an event requires the creation of a vision, operational plans, targeted training of personnel and infrastructure.

Primary morbidity of adolescents in the Vilyui group of districts of the republic of Sakha (Yakutia) in 1996–2021

Abstract

The analysis of the primary morbidity of the adolescent population of the districts/uluses included in the Vilyui group (Verkhnevilyuisky, Vilyuisky, Mirninsky, Nyurbinsky and Suntarsky) has been carried out. First, we analyzed the morbidity data in the indicated uluses in a comparative aspect with all the administrative-territorial formations of the republic. At the same time, the levels of morbidity for certain classes of diseases were revealed in dynamics for the periods under consideration. It was found that, in terms of the incidence of adolescents, in general, the Suntarsky, Nyurbinsky and Verkhnevilyuisky uluses had a relatively better position, and the Vilyui ulus had a relatively worse position. In the Vilyui ulus, relatively high levels of primary morbidity are noted in 12 classes of diseases out of 17, while in the Nyurbinsky ulus – one each.

Regional features of the alcohol situation and health and demographic consequences

Abstract

Purpose: based on the results of the analysis of official statistics of medical and demographic indicators and data from a questionnaire survey on alcohol consumption, to highlight the features of alcoholization in cities and villages of the Republic of Mari El with the rationale for priority areas for reducing negative consequences. Methods. Processing of data on alcohol consumption, medical and demographic indicators, as well as the results of a survey of 403 people according to the AUDIT test was carried out using descriptive statistics, Pearson’s chi-square test, ANOVA multivariate analysis of variance, and Spearman’s correlation analysis. Results. About 80% of the respondents used alcoholic beverages, one in five of them in excessive quantities. With an increase in the age group of the population, the consumption of low-alcohol drinks was replaced by strong ones, accompanied by a decrease in the proportion of teetotalers and a reduction in consumption in alcohol equivalent. An increase in the level of education had a positive effect on the proportion of non-drinkers and the strength of drinks consumed. Alcohol consumption among rural residents, compared with urban residents, was characterized by a high proportion of strong alcoholic beverages, with a greater prevalence of vodka and moonshine. Mortality and socially significant morbidity rates correlated with a delayed effect with the level of alcohol sales and consumption. Conclusion. One of the leading ways to reduce the risk to public health from alcohol is to reduce the consumption of strong drinks, which have a long period of negative effects.

Analytical review of court decisions on claims of patients to medical organizations

Abstract

Introduction. The professional activity of medical workers is regulated by legislation aimed at reducing risks for patients. In recent years, the number of patient lawsuits against medical institutions has increased significantly. The main reasons for dissatisfaction with the quality of medical care are errors in medical documentation, medical errors and non-compliance with the principles of medical ethics. The analysis of judicial practice is important for optimizing court costs and resolving conflicts in a pre-trial manner.The purpose of the study is to analyze the court decisions made on the claims of citizens to a medical organization on the quality and result of medical care.Methods. The study analyzed 56 decisions of the courts of general instance issued over five years – from 2018 to 2022, in court proceedings in which clinical hospitals appeared as a defendant.The main conclusions that can be distinguished from the analysis are that most cases involving compensation for moral damage from medical institutions are based on incorrect treatment, incorrect diagnosis or violation of the right to preferential medical services. In some cases, the courts take the side of the defendant, reject the claim or conclude a settlement agreement. In cases of defeat in court, hospitals pay moral compensation to plaintiffs in various amounts, from 2 thousand to 1.3 million rubles.
