Influence of virus load, the beginning and the regimen of specific antiretroviral (AVR) prophylaxis on expression of destructive and adaptive changes in placental bed structures of the uterus during HIV-associated pregnancy is studied. 61 HIV-infected pregnant women from 17 till 41 years were surveyed. It is proved, that in the placental bed specimens collected during SC in HIV-infected women, the focal chronic deciduitis took place which morphologically correlated with prolonged pre-conceptional persistency of infection. Increasing of the virus load led to elevation of inflammatory cells and destructive changes. With increasing duration of AVR prophylaxis and administration of highly active antiretroviral therapy (HAART) the areas of necrosis and fibrosis were enlarged, atrophic changes in the decidual tissue were observed. Statistical analysis has allowed to find out that virus load, duration and the therapeutic regimen determine severity of destructive processes in uterine placental bad and realization of adaptive reactions that promote the development of placental insufficiency during HIV-associated pregnancy.
