先天性胫骨前脱位是一种罕见的骨科疾病,发病率约为每10万新生儿1例。由于患病率极低且相关研究有限,目前在保守治疗无效时尚未建立统一的外科治疗算法。在PubMed, Scopus, eLibrary, CyberLeninka, Web of Science数据库中,按照以下关键词检索当代相关文献:“врожденный передний вывих голени”(先天性胫骨前脱位)、“хирургическое лечение врожденного переднего вывиха голени”(先天性胫骨前脱位的外科治疗)、“врожденный вывих коленного сустава”(先天性膝关节脱位)、 “congenital dislocation of the tibia”(先天性胫骨前脱位)、“surgical treatment of congenital anterior dislocation of the tibia”(先天性胫骨前脱位的外科治疗)、“congenital dislocation of the knee joint”(先天性膝关节脱位)。分析了150篇文献,其中筛选出40篇,包含有关先天性胫骨前脱位外科治疗的数据。现代临床实践中应用了多种外科方法,但其疗效仍存在争议。本文对现有的先天性胫骨前脱位外科治疗方法进行了分析,探讨了其优点和缺点,并指出了需要进一步研究的关键问题。手术矫正大多数情况下应用于难治性及合并综合征的病例,这类患者可能出现膝关节僵硬、胫骨屈曲挛缩以及伸肌装置功能减弱。因此,在规划应用外科治疗方法时,必须充分考虑患者的个体特征,例如脱位本身的严重程度,以及伴随的全身性和骨科疾病。
