Современные сахароснижающие средства в лечении сахарного диабета 2-го типа

Аннотация
Достижение и поддерживание стабильного метаболического контроля, начатого с момента установления диагноза и продолжающегося в течение всей жизни пациента, является главной задачей лечения СД.Отечественные критерии компенсации на протяжении многих лет постоянно менялись, но понимание значимости гипергликемии в механизмах развития сосудистых осложнений нашло отражение в современных критериях компенсации углеводного обмена при СД 2-го типа. Критерии изменились в сторону приближения показателей углеводного обмена у больных СД к значениям гликемии, наблюдаемым у практически здоровых лиц.На первом этапе используется немедикаментозная терапия, которая включает в себя диетотерапию и увеличение физической активности.Если правильное соблюдение диеты в сочетании с физическими нагрузками не приводит к компенсации углеводного обмена, то следующим этапом следует назначение пероральных сахароснижающих препаратов (ПССП). Механизмы действия сахароснижающих препаратов различны, но в целом направлены на устранение трех основных метаболических нарушений, приводящих к гипергликемии: нарушение секреции инсулина поджелудочной железой, периферическая инсулинорезистентность, избыточная продукция глюкозы печенью. Дополнительный механизм действия – замедление всасывания глюкозы в тонкой кишке и за счет этого уменьшение постпрандиального подъема гликемии. В настоящее время отечественная диабетология обладает полным арсеналом терапевтических средств для лечения СД 2-го типа, включая самые современные ПССП и инсулины, а также средства самоконтроля гликемии и структурированные программы обучения больных. Все это позволяет обеспечить хороший гликемический контроль и высокое качество жизни больных, отдалив развитие тяжелых хронических осложнений заболевания.
