Abstract
Женщины в 30 раз чаще, чем мужчины страдают от инфекций мочевых путей (ИМП). Каждый год около 15% сексуально активных женщинотмечают развитие не менее 1 клинического эпизода ИМП, а в течение своей жизни до 60% всех женщин сталкиваются с этой проблемой. У 1 из 4 пациенток этой группы в течение 1-го года развивается рецидивирующая ИМП. ИМП нередко наблюдается у небеременных женщин пременопаузального возраста. При этом наиболее частым ее клиническим проявлением является цистит, характеризующийся внезапным появлением дизурии, поллакиурии, ургентного мочеиспускания и тазовой боли. Диагноз подтверждается наличием в моче, полученной из средней порции, более 10 лейкоцитов в 1 мм3 и ростом более 103 КОЕ/мл уропатогенов при бактериологическом исследовании. ИМП считается рецидивирующей, если в течение 12 мес развиваются не менее 3 клинических эпизодов, подтвержденных результатами урокультуры.Клиническое проявление заболевания зависит от типа уропатогенов, их вирулентности и резистентности к антимикробным агентам, а также в известной степени от общего состояния организма. Многообразие специфических факторов, взаимодействующих между макроорганизмом и внедрившимися микроорганизмами, определяет прогресс заболевания. При остром цистите часто пациентка испытывает значительные неудобства в связи с неожиданным началом, быстрым течением заболевания и социальной дезадаптацией, обусловленной дизурией. Каждый эпизод ИМП сопровождается клиническими проявлениями цистита в среднем в течение 6,1 дня, ограничением активности и потерей трудоспособности на протяжении 2,4 дней, из которых около 0,4 дня требуется постельный режим. Основные факторы риска развития рецидивирующей ИМП хорошо известны. У молодых женщин наиболее важными факторами риска заболевания острым циститом являются наличие в анамнезе эпизодов ИМП и предшествующая сексуальная активность; применение спермицидов также существенно увеличивает этот риск. При наличии ИМП у матери первые клинические проявления ИМП у дочери наблюдаются нередко в раннем детском возрасте. В постменопаузальном возрасте риск ИМП возрастает у больных сахарным диабетом; кроме того, отмечено, что недержание мочи, эпизоды ИМП перед наступлением менопаузы и несекреторный статус крови наиболее часто связаны с высоким риском развития рецидивирующей ИМП.Проведено сравнение клеток эпителия, полученных у больных рецидивирующей ИМП с подобными клетками лиц контрольной группы; при этом выявлена более активная адгезия патогенных бактериальных штаммов к эпителиальным клеткам у больных ИМП. Выявление и изучение факторов риска развития ИМП позволяет нам совершенствовать методы ее лечения и профилактики.
