Abstract
Применение антибактериальных препаратов неразрывно связано с проблемой устойчивости к ним микроорганизмов.Первый комбинированный препарат, содержащий амоксициллин и клавулановую кислоту, появился в клинической практике в 1981 г. и с тех пор занимает одно из центральных мест в лечении инфекций дыхательных путей.В результате сочетания с клавулановой кислотой амоксициллин восстановил свою природную активность против Haemophilus influenzae, Moraxella сatarrhalis, стафилококков, гонококков, неспорообразующих анаэробов. Помимо того, спектр антимикробной активности амоксициллина/клавуланата расширился за счет действия на ряд грамотрицательных микроорганизмов (Klebsiella spp. и др.) с природной устойчивостью к пенициллинам.Амоксициллин/клавуланат обладает высокой (90%) биодоступностью при приеме внутрь, создает высокие концентрации в легких и плевральной жидкости, быстро проникает в мокроту и накапливается в слизистой оболочке бронхов, создавая концентрации, значительно превышающие минимальную подавляющую концентрацию чувствительных микроорганизмов, включая продуцирующие β-лактамазу штаммы H. influenzae и M. catarrhalis.Амоксициллин/клавуланат отличается хорошей переносимостью, нежелательные явления развиваются относительно редко (в 8–14% случаев), в большинстве случаев носят транзиторный характер и не требуют отмены препарата. Среди них чаще всего фигурируют диарея (9%), тошнота (3%), кожная сыпь (3%), изменения со стороны гепатобилиарной системы .Широкий спектр антимикробной активности, привлекательная фармакокинетика и приемлемый профиль безопасности позволили амоксициллину/клавуланату стать одним из наиболее популярных антибактериальных препаратов, применяемых в лечении большого числа инфекционных заболеваний. Одним из основных показаний для назначения антибиотика является инфекционное обострение хронической обструктивной болезни легких (ХОБЛ).
