Abstract

Одной из основных причин летальности при аутоиммунной патологии, прежде всего системной красной волчанке (СКВ) и ревматоидном артрите (РА), являются сердечно-сосудистые катастрофы (инфаркт миокарда – ИМ, инсульт, внезапная сердечная смерть), обусловленные ранним развитием и быстрым прогрессированием атеросклеротического поражения сосудов [1–11]. Метаанализ 17 проспективных исследований (n=92 000) свидетельствует о повышении уровня сердечно-сосудистой смертности при РА на 60% по сравнению с общей популяцией, причем за последние 50 лет отсутствует тенденция к его снижению [12]. Увеличение риска сердечно-сосудистой летальности показано и при СКВ (4–76%), особенно у пациентов молодого и среднего возраста. Риск развития ИМ у больных СКВ в 5–9 раз выше, чем в общей популяции, а у женщин 35–44 лет – более чем в 50 раз [8, 9, 13]. Относительный риск (ОР) развития сердечно-сосудистых осложнений (ССО) у пациентов с РА значительно выше, чем в общей популяции, и у больных, страдающих заболеваниями с доказанно высоким кардиоваскулярным риском (сахарный диабет – СД, артериальная гипертензия – АГ) [14]. При этом высокий риск ССО при РА регистрируется на 10 лет раньше, чем у лиц без ревматических заболеваний (РЗ). Его нарастание отмечено уже в дебюте заболевания и ассоциировано с серопозитивностью по ревматоидному фактору (РФ) и/или с гиперпродукцией антител к циклическому цитруллинированному пептиду (АЦЦП)
