Abstract
Несмотря на убедительные доказательства того, что при лечении артериальной гипертонии (АГ) снижается сердечно-сосудистая заболеваемость и смертность, контроль за артериальным давлением (АД) повсеместно остается неудовлетворительным [1]. Как показали результаты последних клинических исследований, хуже всего обстоит дело с контролем систолического АД (САД) во всей популяции лиц с АГ, в особенности у больных с сопутствующим сахарным диабетом (СД) типа 2 (рис. 1), при котором целевой уровень АД, рекомендуемый мировыми экспертами в области клинической гипертензиологии, еще ниже – около 130/80 мм рт. ст. [1, 2]. Первостепенная важность создания новых и лучших стратегий для снижения АД у лиц, страдающих СД, обусловлена большим риском развития осложнений, связанных с этим заболеванием, на всех этапах сердечно-сосудистого континуума.Профилактика сердечно-сосудистых осложнений у больных СД должна быть направлена на весь комплекс факторов риска, включая гипотензивную, сахароснижающую, гиполипидемическую терапию, лечение дезагрегантами и отказ от курения. С целью дальнейшего изучения влияния интенсивных режимов гипотензивной и сахароснижающей терапии на риск развития сердечно-сосудистых осложнений у больных СД типа 2 было предпринято новое крупномасштабное рандомизированное исследование Action in Diabetes and VAscular disease – preterax and Diamicron MR Controlled Evaluation (ADVANCE).
