Abstract
Признаки почечного поражения – микроальбуминурия и протеинурия – обозначены в рекомендациях European Society of Hypertension пересмотра 2007 г. в качестве одного из приоритетных показаний к назначению блокаторов рецепторов ангиотензина II (ATII) [1]. Способность этих препаратов замедлять прогрессирование почечной недостаточности обоснована с патогенетической точки зрения и объясняется в первую очередь устранением последствий взаимодействия АТII с рецепторами типа 1 – не только собственно вазоконстрикции, но и локальной почечной продукции ряда профиброгенных медиаторов и факторов роста, в том числе моноцитарного хемотаксического протеина типа 1 (MCP-1) и трансформирующего фактора роста-b (TGF-b), а также маркеров эндотелийзависимого звена гемостаза, например ингибитора активатора плазминогена типа 1 (PAI-1) [2–5]. Устранение провоцируемых АТII нарушений функции эндотелия представляет собой одну из центральных составляющих органопротекторного действия блокаторов рецепторов АТII, во многом определяющую и возможность предупреждения с их помощью хронической почечной недостаточности [6, 7]. Коррекция эндотелиальной дисфункции под действием блокаторов рецепторов АТII, в частности ирбесартана, многократно продемонстрирована в специально планируемых клинических исследованиях.
