Abstract
Считается, что болевые проявления у лиц пожилого возраста имеют отличительные черты. Клинические наблюдения показывают, что с возрастом чувствительность к боли снижается, так же как и другие виды чувствительности.Несомненно, клинический паттерн хронических болевых синдромов, по-видимому, меняется с возрастом. Но означает ли это, что болевые пороги или толерантность к боли различны у пожилых и молодых индивидуумов, а связанная с болью дезадаптация усиливается в процессе старения?Многие пожилые люди с болевыми синдромами не получают адекватную терапию. Приблизительно 40–80% пожилых в домах престарелых и 16–27% госпитализированных пациентов не получают лечения от боли. Между тем хроническая боль является одной из главных причин дезадаптации, снижения функциональных возможностей пациента.Фармакотерапия в целом у лиц старшей возрастной группы имеет некоторые особенности. Люди преклонного возраста употребляют почти в 3 раза больше лекарств, чем представители остальных возрастных групп, причем женщины используют их в 2 раза чаще, чем мужчины. При этом более 50% всех пожилых людей не соблюдают предписанный режим приема лекарств, а примерно 25% делают ошибки, которые с большой степенью вероятности приводят к ятрогенным заболеваниям, обусловленным действием лекарственных средств.Частота побочных эффектов лекарственных средств у пожилых более чем в 2 раза выше, чем у молодых, что связано с изменением метаболизма и экскрецией лекарственных препаратов, а также реакцией на них. Кроме того, на эти особенности могут влиять многочисленные изменения в желудочно-кишечном тракте и снижение функции почек. Все эти факторы требуют при выборе терапевтических доз и режима дозирования фармакологического средства учета особенностей пожилого возраста.
