Abstract
Неспецифический язвенный колит (ЯК) входит в группу так называемых воспалительных заболеваний кишечника. Прогресс в понимании этиопатогенеза данной патологии позволил создать новые средства для лечения. В настоящее время консервативная терапия служит основой лечения воспалительных заболеваний кишечника, а хирургическое вмешательство проводится только в случае развития осложнений или неэффективности консервативной терапии. На сегодняшний день в арсенале врача для купирования обострения ЯК имеются противовоспалительные препараты (аминосалициловая кислота и кортикостероиды), антибиотики и иммуномодуляторы (азатиоприн, 6-меркаптопурин, циклоспорин). Все эти лекарственные средства действуют за счет неспецифического подавления воспалительных процессов, и их применение улучшает состояние и спасает жизни многих и многих больных. Однако их эффективность ограничена, и у каждого из них есть свои побочные эффекты. Теоретически биологические препараты должны были бы быть эффективнее и обладать менее выраженными побочными эффектами за счет более целенаправленного действия на иммунную систему.сегодня в арсенале врача появился новый мощный препарат для лечения тяжелых случаев неспецифического ЯК. Инфликсимаб показал себя эффективным для индукции ремиссии, благодаря ему удалось у большого числа больных уменьшить дозу кортикостероидов или вовсе отказаться от них. Вероятно, в ближайшее время будут проведены исследования, касающиеся качества жизни на фоне применения инфликсимаба, его влияния на вероятность необходимости хирургического лечения, а также получены новые данные относительно иммунопатогенеза этого все еще остающегося загадкой заболевания.
