Abstract
Парентеральное питание, включающее аминокислоты, жировые эмульсии и растворы глюкозы, занимает одно из ведущих мест в системе лечения больных отделений реанимации и интенсивной терапии (ОРИТ). Несмотря на появление новых сред для энтерального и перорального питания, в ряде клинических ситуаций (невозможность адекватного энтерального питания, абдоминальная хирургия) парентеральное питание остается основным, а иногда и единственным средством спасения жизни больных. Наиболее перспективным экономически оправданным способом проведения парентерального питания в настоящее время является применение препаратов "три в одном", содержащих в одном пакете аминокислоты, жировые эмульсии и растворы глюкозы. Технология "три в одном" – это новый подход к реализации парентерального питания для России: только в 2005 г. завершилась регистрация препарата для парентерального питания "три в одном".В настоящее время готовые к использованию препараты "три в одном" в трехкамерном пакете считаются стандартом как для краткосрочного, так и для длительного парентерального питания взрослых пациентов. Наиболее часто применяемым трехкомпонентным препаратом в Европе является кабивен, представляющий собой пакет, состоящий из трех камер, содержащих раствор аминокислот (вамин 18), жировую эмульсию Intralipid и раствор глюкозы. Камеры разделены перегородками, которые перед применением вскрываются, и содержимое пакетов смешивается. Препарат "Кабивен" содержит хорошо известные компоненты, безопасность которых подтверждена как для взрослых, так и для детей разного возраста.
