Abstract
Остеоартроз (ОА – остеоартрит) – гетерогенная группа заболеваний различной этиологии со сходными биологическими, морфологическими и клиническими проявлениями и исходом, в основе которых лежит поражение всех компонентов сустава в первую очередь хряща, а также субхондрального участка кости, синовиальной оболочки, связок, капсулы, периартикулярных мышц. Эти изменения происходят постепенно в результате того, что травмирование хряща при нагрузке превышает скорость репарации. ОА традиционно считался дегенеративным заболеванием суставов, однако в последнее время появляется все больше данных о том, что в его патогенезе немалую роль играет воспаление. Именно поэтому в зарубежной литературе заболевание принято называть "остеоартритом". В целом ОА характеризуется очаговой потерей суставного хряща и центральным и краевым образованием новой костной ткани. ОА приобретает все большую значимость для здравоохранения, поскольку в обществе увеличивается доля пожилых людей, а также людей с повышенной массой тела. Наибольшую медицинскую и социальную значимость имеет ОА крупных суставов нижних конечностей (коленных и тазобедренных) в связи с высокой распространенностью и значительными функциональными нарушениями у этих больных. Лечение больного ОА является преимущественно симптоматическим и в некоторой степени, профилактическим, предупреждающим прогрессирование заболевания с развитием тяжелых функциональных нарушений и необходимостью протезирования сустава. Оптимальное лечение должно включать комбинацию нефармакологических и фармакологических методов.
