Abstract
Лимфопролиферативные заболевания (ЛПЗ) представляют собой большую гетерогенную группу опухолей.Индолентные ЛПЗ хотя и отличаются медленным прогрессированием и относительно длительной выживаемостью, но в силу низкой пролиферативной активности опухолевых клеток проявляют слабую чувствительность к химиотерапии. Имеющиеся методы терапии достаточно редко вызывают полную длительную ремиссию, которая в обязательном порядке сменяется рецидивом, и именно поэтому индолентные ЛПЗ пока сохраняют статус потенциально неизлечимых заболеваний. Лечение индолентных ЛПЗ является сложной задачей, которая в равной степени сочетает борьбу как за длительность жизни пациента, так и за ее качество. Расширение знаний о биологии данного класса опухолей идет чрезвычайно быстро, и успехи в лечении ЛПЗ за последние 10–15 лет очевидны. За этот период было создано и внедрено в клиническую практику много принципиально новых противоопухолевых препаратов для лечения ЛПЗ, возможно больше, чем в других областях онкологии. Хотя все эти препараты, повысив вероятность достижения полных ремиссий, все-таки не дали значимого увеличения продолжительности жизни больных индолентными ЛПЗ, тем не менее качество их жизни в последнее десятилетие, безусловно, улучшилось. Одно из наиболее успешных "приобретений" клинической практики за этот период – широкое применение структурных аналогов пурина, в частности флударабина фосфата, препарата, который в равной степени существенно увеличил как длительность, так и качество жизни пациентов с индолентными ЛПЗ.
