基于数字技术的重症监护病房微生物学监测与抗菌耐药性评估体系

详细
论证。医疗相关感染是当代卫生保健体系的一项严重问题,其中以多重耐药菌株为主要致病因子。重症监护病房中Klebsiella pneumoniae的广泛流行尤具临床关注意义,因为在病情危重的患者中,感染多重耐药菌株会加重基础疾病进程、延长治疗时间并造成显著的经济损失。系统性的微生物学监测以及对患者生物材料中分离菌株抗菌耐药性的评估,是医疗相关感染防控体系的重要组成部分。
目的。基于现代数字技术,对Saint Petersburg两家综合医院重症监护病房与心脏重症监护病房中分离的多重耐药菌株的微生物谱及抗菌耐药性进行比较分析。
方法。对Saint Petersburg两家综合性医院重症监护病房和心脏重症监护病房2024年度的微生物学监测结果进行了回顾性分析。所得数据采用计算机分析程序WHONET和在线平台 AMRCloud进行处理。
结果。重症监护病房和心脏重症监护病房中分离的微生物以ESCAPE病原体占主导,主要来源为痰液及支气管肺泡灌洗液。重症监护病房菌株的耐药水平显著高于 心脏重症监护病房。为分析与机械通气相关性肺炎的流行情况,计算了分层流行病学指标,其结果与文献数据一致。
结论。定期开展的微生物学监测是控制住院医疗相关感染致病菌及其抗菌耐药性动态的主要工具之一,它能够及时应对不断变化的流行病学形势,了解微生物结构、形成的菌株联合及其对抗菌药物的耐药性。由于该工作与大量数据的快速分析密切相关,亟需开发并将新的数字技术应用于医院信息系统中。
