北极俄罗斯地区急性缺氧前后间歇性低氧训练对心脏效应的总体与综合分析

背景。北极地区环境生理学与医学的一项独特且研究较少的问题是,在寒冷季节发展性低氧血症条件下心脏功能的研究。这正是本研究的主题。
研究目的。研究北极俄罗斯地区冬季健康人群在急性缺氧前后,心脏生物电过程的变化特征,以及间歇性常压低氧训练对急性缺氧反应的整体类型影响。
材料与方法。本研究纳入29名男性(平均年龄20.2±0.31岁),对其进行急性缺氧测试(12.3% O2, 持续20分钟),记录血氧饱和度(SpO2%),以及3个ECG振幅和时间参数的变化。这些参数在测试前和间歇性常压低氧训练后的第1天、第7天和第16天进行监测,同时确定心脏时-肌动力效应的综合值。
结果。在对照组急性缺氧条件下,P1II(p=0.005)和P-Q(p=0.006)增加,而RII、T1II、R-R和Q-T显著减少(p<0.001)。与对照组相比,间歇性常压低氧训练后急性缺氧条件下的T1II偏差从第1天(p=0.030)到第16天(p<0.001)显著降低,而P1II在第16天显著下降(p=0.020)。T1II和Q-T的变化始终与SpO2%的变化相关。在所有急性缺氧测试中,中等反应的发生率为42.5–47.1%,高反应率为24.1–29.9%,低反应率为27.6–28.7%。从第7天起,急性缺氧诱导的心脏极性时-肌动力反应的综合均值显著高于对照值。
结论。低氧训练能够减弱急性缺氧时右心房生物电活动的变化,并减轻快速终末复极化阶段中心室肌活动的抑制。同时,心室收缩对缺氧发展的依赖性增强。尽管不同极性的心脏时-肌动力效应的比例关系未发生显著变化,但其综合平均值有所增加。
