Экзосомы стволовых клеток в патофизиологии сердечно-сосудистых заболеваний

Аннотация
Обсуждается современное состояние терапевтического потенциала внеклеточных везикул, который зависит от способов их выделения, их состава, а также характеристик везикулярных и невезикулярных компонентов. Известно, что повреждение миокарда, особенно в результате острого инфаркта миокарда, приводит к необратимой гибели кардиомиоцитов, саркомеров и, в конечном итоге, к сердечной недостаточности. Сердце взрослого человека имеет ограниченную регенеративную способность, вследствие чего стимуляция эндогенных репаративных и регенеративных потенциалов с использованием клеточной терапии имеет потенциальные перспективы. При этом польза от инъекции стволовых клеток и клеток-предшественников в поврежденном миокарде опосредована секретируемыми ими факторами. В частности, экзосомы — наноразмерные секретируемые внеклеточные везикулы эндосомального происхождения — стали ключевыми сигнальными органеллами в межклеточной коммуникации и в настоящее время рассматриваются как ключевой регенеративный компонент секретомы стволовых клеток и клеток-предшественников. Экзосомы, высвобождаемые из кардиальных эмбриональных и мезенхимальных стволовых клеток, резидентных стволовых и прогениторных клеток (включая специфическую подгруппу клеток кардиосфер), индуцированных плюрипотентных стволовых клеток и выделенных из них кардиомиоцитов, обладают кардиопротекторными, иммуномодулирующими и репаративными способностями. Еще одним перспективным направлением использования экзосом является их применение в направленном транспорте лекарственных средств в липидоподобных наноконтейнерах и внеклеточных везикулах. Так как искусственные носители лекарств, включая липосомы и наночастицы на основе липидов, ограничены потенциальной токсичностью, иммуногенностью и неспособностью нацеливаться на определенные органы, экзосомы имеют хорошие перспективы в качестве потенциальных носителей лекарственных препаратов. Соединения могут транспортироваться как внутри экзосом, так и на их поверхности. В целом секретируемые внеклеточные везикулы, особенно экзосомы, могут считаться ключевым функциональным компонентом секретомы стволовых клеток и особенно кардиогенных клеток-предшественников (мезенхимальных стволовых клеток, эндогенных кардиальных клеток-предшественников, кардиосфер, эмбриональных стволовых клеток костного мозга, индуцированных плюрипотентных стволовых клеток костного мозга). Они продемонстрировали терапевтическую эффективность в доклинических моделях при изучении сердечно-сосудистой патологии.
