采用腹部减压法预防心脏过度负荷于心脏发育小异常篮球运动员 (非随机对照临床试验)

论证。受专项选材机制影响,篮球运动员更易出现心脏过度负荷。因此,在训练过程中应特别重视引入能够提高心血管系统适应能力的干预方法。
目的。确定在伴有心脏发育小异常的篮球运动员训练中应用腹部减压方法,对降低心脏过度负荷风险的有效性。
方法。开展了对 53 名篮球运动员心血管系统状态的研究,受试者均在P.F. Lesgaft National State University of Physical Education及V.P. Kondrashin Olympic Basketball Training Center进行训练。纳入标准:男性篮球运动员,年龄18–25岁,运动年限不少于5年,具有运动健将候选人资格。未纳入标准:无心脏发育小异常者,以及在初次检查时存在潜在危险性心律失常者。排除标准:腹部减压耐受性差或干预中断。小异常通过超声心动图确诊。实验组(n=12)在一年内共接受6个疗程的腹部减压治疗。对照组(n=12)未实施腹部减压治疗。对照组与实验组篮球运动员均分别在一般准备期及过渡期接受检查,内容包括:24小时Holter动态心电监测、血清心肌抗原测定、皮质醇与睾酮检测并计算同化指数、总白蛋白与有效白蛋白测定并计算毒性指数,以及氧化脂蛋白测定。
结果。24名篮球运动员(45.3%)被诊断为心脏发育小异常。研究证实腹部减压方法在预防心脏发育小异常篮球运动员心脏过度负荷方面具有较高有效性。其主要效果在于提高心血管系统的适应能力并预防心脏过度负荷,这主要通过降低内源性中毒水平实现,同时在较小程度上有助于代谢过程的正常化。未发现腹部减压疗程对氧化应激程度产生明显影响。运动员在腹部减压过程中未出现不良反应。
结论。腹部减压在伴有心脏发育小异常的篮球运动员中具有较高应用潜力,可用于降低内源性中毒水平并调节同化/分解代谢比例。在此基础上,可观察到其促进心血管系统对训练与比赛负荷的适应性提高,从而降低心脏过度负荷风险。
