背景。血管运动性鼻炎(Vasomotor Rhinitis, VR)是常见的耳鼻喉疾病之一,其典型表现为鼻腔阻塞,不仅引起明显的生理不适,还可导致心理情绪问题和社会适应障碍,显著降低患者生活质量。目前,冷等离子体消融(cold plasma coblation)是治疗VR的最有效现代方法之一,可选择性破坏下鼻甲的血管网络。术后VR患者康复的有前景技术是低强度激光治疗 (Low-Intensity Laser Therapy, LILT)与经颅电刺激(Transcranial Electrical Stimulation, TES)的联合应用,两者具有显著的病理生理调节作用。
目的。评估在VR患者冷等离子下鼻甲血管切除术后联合应用LILT与TES的临床疗效。
方法。材料与方法: 进行前瞻性、对照性、随机化比较研究,共纳入137例VR患者,采用简单随机法分为4组。第一组(对照组,n=34)仅接受基础治疗。第二组(比较组1,n=35)在基础治疗的基础上接受LILT。第三组(比较组2,n=34)在基础治疗的基础上接受TES。第四组(实验组,n=34)在基础治疗的基础上联合应用LILT与TES。通过患者主诉变化、反应性炎症表现、前鼻主动鼻阻力测定及SNOT-22问卷评分评估术后物理因子疗法的效果。
结果。物理因子的额外应用促进了VR患者症状的回退,其中实验组改善最为显著。客观参数的变化与VR主观症状的动态一致。联合LILT与TES可最大程度恢复前鼻主动鼻阻力测定参数和黏液纤毛传输时间。在术后黏膜反应性炎症动态变化中,观察到物理因子对评估指标具有积极作用,实验组取得了最佳结果。
结论。联合应用LILT与TES可加速VR术后临床表现及反映鼻腔黏膜功能状态的客观参数的恢复。综合物理治疗的应用可显著促进疾病临床表现的消退,并最大程度改善患者生活质量。
