3D视频重建康复系统训练对膝关节全置换术后早期功能恢复的影响

论证。全膝关节置换术(total knee arthroplasty, TKA)是治疗Ⅲ–Ⅳ期变形性膝关节骨关节炎的“金标准”,然而在术后早期康复阶段仍有多达20%的患者出现持续性关节功能受限、步态障碍及生活质量下降。将具有增强现实元素的步行康复技术整合至康复过程,使训练更贴近真实生活情境,是当前康复医学的重要任务。
目的。在《国际功能、残疾和健康分类》(International Classification of Functioning, Disability and Health)框架下,评估在TKA后早期康复期间使用“D-Wall”系统(DIH S.r.l., Florence, Italy)的训练效果。
方法。进行一项前瞻性、随机、对照研究,纳入60名行初次TKA后进入医学康复第二阶段的患者,平均年龄68.2±6.5岁。患者采用简单随机法分为两组:实验组(n=30)在标准康复方案基础上增加“D-Wall”系统训练,对照组(n=30)仅接受标准综合康复方案。功能状态评估分别在医学康复开始前和为期10天的医学康复课程结束后实施。
结果。10 天康复结束后,与对照组相比,实验组患者出现统计学显著改善:疼痛视觉模拟评分降低42%(实验组4.1±1.2分;对照组6.3±1.5分,p<0.01);膝关节屈曲活动度增加 25°(实验组115±8°;对照组90±10°,p<0.001);根据“C-Mill”步态诊断平台,步行速度提高35%(实验组0.92±0.15 m/s;对照组0.68±0.12 m/s,p<0.01);社会–生活适应能力提高28%(p<0.05)。
结论。将具备3D视频重建与动态步态生物反馈功能的“D-Wall”康复系统纳入TKA术后早期康复方案,有助于加速运动功能恢复、改善步态并提高患者的生活质量。
