外周动脉疾病(Peripheral Artery Disease, PAD)与糖尿病(Diabetes Mellitus, DM)是当前严重的医学与社会问题,与心血管并发症、截肢及致残风险显著增加密切相关。两种疾病的共病显著降低患者的生存率与肢体保留率,加重下肢缺血程度,并提高足部组织营养障碍及糖尿病足综合征的发生率。现代PAD与DM的治疗基于多学科综合策略,其中治疗性体疗医师具有重要地位。其职责涵盖优化身体活动、预防并发症、慢性创面护理以及血管重建术后的康复。本文综述了当前治疗中应用的主要体疗方向,包括用于间歇性跛行的监督性运动治疗(Supervised Exercise Therapy, SET)、术前身体准备方案、早期活动及截肢或溃疡愈合后的运动功能恢复。重点讨论了足部病变减压策略、物理治疗方法(如激光疗法、磁疗)以及针对DM患者的个体化运动处方。同时指出SET推广中存在的障碍,包括项目可及性不足、专业人员短缺及患者转诊路径不完善等问题,并强调开展慢性创面管理教育及康复医师认证培训的重要性。将治疗性体疗方法纳入PAD与DM患者的综合治疗体系,有助于改善肢体功能状态,降低并发症发生率并提高生活质量,这一点已得到临床研究数据的证实。
