Гнёздная алопеция детского возраста: обсуждение особенностей проявления и современных методов лечения

Аннотация
Гнёздная алопеция ― хроническое рецидивирующее заболевание аутоиммунной природы, характеризующееся выпадением волос на ограниченном участке или по всей поверхности кожного покрова без формирования рубцовой ткани в очагах поражения. До 0,2% населения земного шара страдает гнёздной алопецией в настоящее время, а у 1,7% людей это заболевание возникало в какой-либо момент их жизни. Частота возникновения гнёздной алопеции у детей в последнее время стремительно растёт.
В патогенезе главная роль отводится потере иммунной привилегии волосяного фолликула в результате влияния генетических и различных внешних факторов. Фолликулы становятся уязвимыми для атаки собственными Т-лимфоцитами, и происходит выпадение волос. Выявлена коморбидность гнёздной алопеции с некоторыми аутоиммунными и аллергическими заболеваниями.
Диагноз выставляется на основании результатов трихоскопии, при этом клинические проявления у детей имеют некоторые особенности в сравнении со взрослыми.
В настоящее время имеется несколько многообещающих препаратов, однако количество исследований (относительно педиатрических пациентов) крайне ограничено. Согласно результатам ряда научных работ, глюкокортикостероиды и ингибиторы янус-киназ, не одобренные в настоящий момент Управлением по санитарному надзору за качеством пищевых продуктов и медикаментов США (FDA), показали свои эффективность и безопасность в лечении гнёздной алопеции у детей.
