Роль витамина D в патогенезе иммуновоспалительных дерматозов

В последние годы витамин солнечного света стал крайне популярным и едва ли не обязательным к употреблению, в особенности из-за его плейотропных эффектов, хотя ещё недавно его применение ограничивалось профилактикой развития патологии костной системы, в частности рахита у детей. Будучи фактически прогормоном стероидной природы, витамин D участвует в механизмах эндокринной, аутокринной и паракринной регуляции организма. Фармакотерапевтический ренессанс кальциферола связан с открытием рецепторов к витамину D в большинстве клеток и тканей организма, а обнаружение ферментов, синтезирующих активную форму витамина D в коже, привело к возобновившемуся интересу и широкой дискуссии в дерматологическом сообществе. Действительно ли роль некостных эффектов кальциферола, главным образом его роль в патогенезе заболеваний кожи, с точки зрения доказательной медицины обоснована, а тенденция на потребление витамина D безопасна?
В данной статье представлены наиболее актуальные сведения о роли дефицита витамина D в механизмах развития иммунного ответа при некоторых дерматозах. Помимо обобщения костных и внекостных функций витамина D на макроорганизм детально обсуждаются механизмы формирования некоторых наиболее распространённых дерматозов, таких как псориаз, атопический дерматит и витилиго. В обзоре детально рассмотрены биологические эффекты витамина D в коже. В статье проанализирована правомерность использования препаратов на основе витамина D и их эффективность в дерматологической практике.
