THE SPEED METHOD AND QUALITY: ARE THEY COMPATIBLE IN DISTRICT THERAPEUTIST WORK?

Abstract

The present system of financing of polyclinics the remuneration of labor of district therapeutists depending on number of received patients and related to condition the regulation of 15 minutes for one patient are not consistent with high quality of work. The occurrence of several diseases, cognitive disorders, depression (spread especially among patients of older age) need more time to contact with patient. The additional time is needed to implement the preventive activities and many other functional duties including making up of big amount of various documents. To implement this entire staff within the framework of present regulations is impossible. There is no way to finance the work of district therapeutist according the number of received patients. Under such an approach, it turned out that the more is the number of patients the better since more money the insurance companies will transfer to polyclinics and this will positively impact the physician financial situation. The opposite is positive - the lesser number of patients the better. This characterizes the effective work of physician and better implementation of preventive and curative measures. The present method of speed method of work of district therapeutist undoubtedly has a negative impact on population health and results in the emotional burning-out of physicians. The replacement of present system of health financing of primary health care activities is needed. The most part of physicians who participated in the survey propose the comeback to poll financing.
