Распространенность психических нарушений у больных системной красной волчанкой: связь с активностью заболевания и сопутствующими хроническими расстройствами

Аннотация

Цель исследования. Оценка влияния активности болезни и сопутствующих хронических расстройств на распространенность психических нарушений у больных системной красной волчанкой (СКВ), наблюдающихся в Институте ревматологии РАМН.
Материалы и методы. В исследование были включены 115 пациентов с достоверной СКВ в возрасте 34 (24; 45) лет (медиана и интерквартильный размах). Продолжительность болезни составила 8 (4; 17) лет. Активность СКВ оценивали с помощью шкалы SLEFAI. Атеросклероз определяли с использованием ультразвуковой допплерографии артерий. Психические нарушения диагностировались психиатром в соответствии с МКБ-10 и использованием ряда психиатрических и психологических шкал.
Результаты. Психические нарушения выявлены у 76 (66%) из 115 пациентов. Преобладали тревожно-депрессивные расстройства (83%): депрессивные эпизоды (40%), расстройства адаптации (24%), генерализованное тревожное расстройство (10%) и дистимия (9%). Выраженные когнитивные нарушения выявлены у 7% пациентов. Не выявлено различий по возрасту, полу, длительности и степени активности СКВ, длительности и кумулятивной дозе глюкокортикоидов (ГК) и цитотоксиков у пациентов с психическими нарушениями и без таковых. Значимо чаще у пациентов с психическими нарушениями выявлялся атеросклероз (23 и 8% соответственно). У всех пациентов с инфарктом миокарда (ИМ) в анамнезе имелись психические нарушения. При антифосфолипидном синдроме (АФС) распространенность психических нарушений увеличивалась (85 и 49% соответственно, p < 0,05).
Заключение. Психические нарушения часто встречаются у больных СКВ (у 66%). Преобладают тревожно-депрессивные расстройства (у 83%). На распространенность психических проявлений не влияют пол, возраст, длительность и активность СКВ, доза применяемых ГК и цитотоксиков, но существенное влияние оказывают сопутствующие заболевания - атеросклероз, ИМ и острое нарушение мозгового кровообращения в анамнезе, а также АФС и синдром Шегрена.
