Системная красная волчанка и антифосфолипидный синдром: вчера, сегодня, завтра

Аннотация
Иммуновоспалительные (аутоиммунные и аутовоспалительные) ревматические заболевания – не только весьма распространенные и тяжелые хронические воспалительные заболевания, но и «модели» для изучения фундаментальных механизмов патогенеза и подходов к фармакотерапии других заболеваний, связанных с развитием аутоиммунитета и/или аутовоспаления. Неконтролируемое воспаление, приводящее к гиперкоагуляции, составляет основу «тромбовоспаления» (thromboinflammation), которое рассматривается как универсальный патогенетический механизм органной патологии при иммуновоспалительных ревматических заболеваниях, а также при COVID-19 (Coronavirus disease 2019) и атеросклеротическом поражения сосудов (атеротромбоз). Особенно важную роль тромбовоспалительные механизмы играют в развитии системной красной волчанки и антифосфолипидного синдрома, в изучение которых большой вклад внесли российские ревматологии под руководством академик а Валентины Александровны Насоновой. Обсуждаются современные представления об общности патогенетических механизмов тромбовоспаления при системной красной волчанке и антифосфолипидном синдроме, значение этих исследований в период пандемии COVID-19 и перспективы антитромботической и противовоспалительной терапии.
