Дисфункция эндотелия при аксиальном спондилоартрите, ассоциированном с воспалительным заболеванием кишечника

Аннотация
Спондилоартриты — группа воспалительных заболеваний опорно-двигательного аппарата, включающая аксиальную и периферическую формы. Современная классификация этих заболеваний обусловливает диагностическую важность известной ассоциации между спондилоартритом и воспалительными заболеваниями кишечника (болезнью Крона и язвенным колитом). Согласно многим крупным исследованиям как при аксиальном спондилоартрите, так и при воспалительных заболеваниях кишечника продемонстрировано повышение кардиоваскулярного риска, однако механизмы, лежащие в его основе, не вполне ясны. Исследователи предполагают, что одним из возможных путей вовлечения сердечно-сосудистой системы в течение этих заболеваний является формирование эндотелиальной дисфункции сосудов, развивающейся при многих воспалительных и невоспалительных состояниях. При этом нарушение функций эндотелия необходимо анализировать многомерно из-за его участия в регуляции множества процессов, таких как обеспечение тонуса сосудов, клеточная миграция, ангиогенез и гемостаз.
В настоящем обзоре рассмотрены последние данные о дисфункции эндотелия при аксиальном спондилоартрите и воспалительных заболеваниях кишечника, отдельно проанализированы исследования этого феномена у пациентов с аксиальным спондилоартритом, ассоциированным с воспалительным заболеванием кишечника. Уделено особое внимание современным лабораторным и инструментальным методам оценки эндотелиальной функции и их потенциальной диагностической значимости в клинической практике.
