脊柱截骨术在治疗椎体外侧切面受损的先天性脊柱侧弯儿童中的应用研究(初步结果)

论证。椎体外侧表面分割异常导致脊柱出现渐进性畸形。唯一有效的治疗方法是各种变体的手术。本文考虑了用于治疗先天性脊柱侧弯和椎体侧表面分割障碍患者的矫正性椎骨切开术的方法。
目的是评估使用矫正性椎骨切开术对患有先天性脊柱侧位表面受损的儿童进行手术治疗的结果。
材料与方法。2014年9月至2020年9月,在H. Turner National Medical Research Center for Children’s Orthopedics and Trauma Surgery进行了一项单中心回顾性研究。研究中包括33名患者:17名女孩和16名男孩。年龄范围为36至211个月。所有患者都接受了单阶段矫正性楔形椎骨切开术。统计学处理是通过使用Wilcoxon t检验比较分布差异的意义。
结果。术前Cobb脊柱畸形的中位数为31°,四分位数间期(IQR)为30.5。根据Cobb标准,手术治疗前脊柱侧弯畸形的中值为29°,IQR为29.5。矫正脊柱侧弯部分的畸形为64%。(干预后的中位值为5°Cobb,IQR为14.5)。矫正胸椎病理性前凸为42%(根据Cobb,干预后的中值为17°,IQR为14.5)。 结果具有统计学意义(p<0.05)。
结论。矫正性楔形椎骨切开术是一种有效的手术治疗,适用于椎体外侧表面分割受损的先天性脊柱畸形儿童。这种方法平均实现了64%的脊柱侧弯畸形矫正和42%的病态前凸矫正。
