多器官功能障碍评分量表在儿科临床实践中的应用:医生是否需要?

详细
论证。文献资料分析表明,目前尚无研究专门分析儿科重症监护与麻醉科医生对在儿童临床实践中应用器官功能障碍严重程度评分量表必要性的看法。
目的:分析俄罗斯联邦儿科麻醉与重症医学科医生对在临床实践中使用多器官功能障碍评分量表评估疾病严重程度的看法。
方法。本研究为观察性、非对照研究,基于俄罗斯儿科麻醉与重症医学协会网站平台,通过自愿匿名问卷收集数据。描述性统计以频数和百分比表示,开放式问题的回答按主要定性特征分类。定性数据的比较采用皮尔逊χ2检验。
结果。共有230名受访者参与调查,受访者中绝大多数为地区临床医院的医生(38.7%)以及从业超过10年的专业人员(69.6%)。最为人熟知且被使用的评分系统包括qSOFA、nSOFA、NEOMOD、pSOFA及Phoenix Sepsis Score。共有77名(33.5%)受访者在临床中定期使用多器官功能障碍评分量表, 其中最常用的是尚未在俄罗斯本土验证的qSOFA评分。
结论。超过60%的儿科麻醉与重症医学科医生并不规律使用多器官功能障碍评分量表,其使用的主要依据往往不是为了确定后续治疗策略,而是出于对死亡风险升高的判断。
